Description:CDC developed a quality improvement assessment tool called the Health Systems Scorecard (HSSC) to assist CDC-funded state and local public health programs in understanding the implementation of evidence-based strategies to manage chronic diseases among partnering primary care practices and health systems. In response to formative evaluation findings, CDC revised and updated the Scorecard. This improvement process was guided by CDC recipients with experience implementing health system assessments. This Coffee Break will provide an introduction to the version 2.0 tool and summarize the improvements made to the Scorecard and supporting resources to enhance usability and applicability to NCCDPHP cooperative agreements. After attending this session, participants will 1) know how to access the HSSC version 2.0, 2) be able to identify differences and enhancements in the updated tool, and 3) learn how to utilize version 2.0 and its supplemental materials.
