Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The decedent placed a ventilation hose into Compartment #4 by opening the ten-inch fill lid of the portal covering. After visually checking the outside of the tanker to confirm the leak location, the decedent removed the portal cover for Compartment #3 and transferred the ventilation hose from Compartment #4 to Compartment #3. The decedent did not open the portal covers for compartments #1 and #2. With the blower hose operational, the decedent entered Compartment #3 on four different occasions AB - once to check the Compartment for the leak location, once to perform grinding to prepare the leak location for welding repair, and twice to perform welding. It is unknown if the decedent conducted air monitoring. After entering Compartment #3 for the fourth time, vapors from Compartment #4 ignited during the welding in Compartment #3 and an explosion occurred. The decedent died at the scene. Recommendations: 1. Employers should identify areas that are classified as a confined space and meet the requirements of a permit-required confined space (PRCS). They should then develop a written PRCS program if employees will be entering the space. The written PRCS should include: 2. a procedure to document boiler operation and tanker cleaning, 3. a requirement for atmospheric monitoring and forced air ventilation in adjacent compartments when conducting hot work, such as welding, within a tanker compartment, and 4. a hot work permit for confined spaces. 5. Employers should review welding practices to ensure proper welding techniques are used. 6. Bulk transport trade organizations should develop a sample tank cleaning procedure that addresses the safety and health issues inherent in the tank cleaning process and welding performed to repair leaks. The three-person crew was in a county park's lift station to replace two pumps. The water was pumped down and the decedent and Coworker #1 went down to the bottom of the manhole to install a mechanical plug into a 26-inch diameter storm sewer pipe that drained into the lift station. The plug was inflated and the water stopped flowing into the station. Coworker #2 was at the top of the 10- x 10-foot manhole monitoring the situation. The decedent and Coworker #1 installed a pump base anchor in the floor directly in front of the mechanical plug they had installed in the sewer line. No blocking or bracing was installed in front of the plug. While the decedent and Coworker #1 were drilling anchor holes in the floor, the wall, sewer pipe and plug exploded into the manhole, striking both employees (Figure 1). The decedent was knocked unconscious and Coworker #1 was thrown across the lift station. Coworkers #1 and #2 lifted the decedent to the 2nd level of the lift station and started CPR. When emergency response arrived, they extricated the decedent from the 2nd level and transported him to a local hospital where he later died. Recommendations: 1. Employers should ensure that workers follow all pneumatic pipe plug manufacturer's safety recommendations and other safety precautions relevant to the safe installation and use of pneumatic pipe plugs. 2. Employers should develop written confined space programs that contain specific procedures for all tasks to be performed and ensure employees follow the procedures. 3. Employers should institute a Health and Safety (H&S) committee as part of their health and safety program. 4. Employers should provide workers with training in the recognition and avoidance of unsafe conditions and the required safe work practices that apply to their work environments. Looking over his shoulder, the operator saw the warehouse clerk walking up the aisle toward him. The operator yelled at the clerk, made eye contact, and hit the brakes and tried to steer away. The forklift struck the warehouse clerk and pinned him against a metal shelf. First aid was provided while waiting for emergency responders. The victim died on the way to the hospital. CAUSE OF DEATH: Severe blunt chest trauma. RECOMMENDATIONS: 1. Forklift operators must always look in the direction of travel and keep a clear view. 2. Employers must ensure that workers know to stay clear of operating forklifts. 3. Employers should closely supervise new workers to reinforce safe work practices. The 27-year-old victim, a member of the IBEW local, worked for a company that installs cable, telephone, and fiber optic lines. The victim and another worker were at a job site at an amusement park working from the bucket of an elevating aerial lift to install fiber optic lines on an existing cable run on a joint-use pole. A 7.2 kV high voltage power line was located above them on the pole. The victim was operating the controls to position the bucket over the tops of fir trees that had been trimmed so as not to interfere with the power lines. Both workers were looking down at the trees when both the co-worker and the victim simultaneously made contact with the overhead power line. The lift was not an insulated lift. The victim died at the scene and the co-worker was hospitalized with electrical burn injuries. La retroexcavadora tenía estructuras de protección contra volcaduras. Jaime estaba excavando una zanja encima de una inclinación de 10 pies. Los siguientes casos describen cómo dos trabajadores murieron al operar prensas para punzón de metales. Un operario de prensa muere mientras cortaba un pedazo de metal Juan L.* (* No es el nombre real de la víctima), un operario de prensa de metales (troqueladora) de 39 años, murió mientras cortaba un pedazo de metal. Los tornillos de fijación que estaban asegurando el molde estaban desgastados lo que causó que el molde se moviera y que el molde y el cilindro quedaran desalineados. Esto causó que la máquina hiciera un ruido durante uno de sus ciclos. Juan se alejó de la máquina, pero entonces pisó un pedal que comenzó un nuevo ciclo en la máquina. Carlo había sido llamado para reparar la máquina después que un pedazo de metal se había atorado en el sistema de montaje. La norma de la empresa exigía que para componer el atoramiento debían primero desconectar el cilindro y el molde y después quitar el pedazo de metal atorado. Carlo quitó el pedazo de metal atorado con una palanca de hierro y después quito una plancha que cubría parte de la máquina. Esa plancha ayudaba a mantener alineados el cilindro y el molde. La máquina comenzó su próximo ciclo. Cuando el cilindro bajó, no estaba alineado con el molde. El borde del cilindro le pegó al molde. Esto causó que un pedazo pequeño de metal se desprendiera del cilindro y se disparara hacía el pecho de Carlo. Carlo no había puesto bloques, candado ni etiqueta indicando que la máquina estaba siendo reparada. No había constancia del tipo de entrenamiento que la víctima había recibido. Recomendaciones: Los trabajadores deben: Asegurarse que el cilindro y el molde están alineados dentro de la prensa de metales antes de poner a funcionar la máquina. Muere mecánico aplastado por escalera eléctrica: Jorge H.* (* No es el nombre real de la víctima), un mecánico de escaleras eléctricas de 37 años, murió aplastado cuando daba mantenimiento a una escalera. Jorge había quitado la escalera y se había metido adentro del mecanismo de la escalera eléctrica. A un compañero de trabajo se le cayó la caja de circuitos eléctricos de la escalera, lo cual causó una transmisión de electricidad en la escalera. La escalera se empezó a mover. Jorge no pudo salirse antes de que apagaran la escalera. No había candado o etiqueta de advertencia en los controles de electricidad de la escalera. El interruptor para la placa de circuitos que daba electricidad a la escalera tampoco tenía candado o etiqueta de advertencia. No se usaron bloques u otros mecanismos para impedir que la escalera se moviera. Soldador muere al ser aplastado por trituradora: José K.* (* No es el nombre real de la víctima), un soldador de 52 años, fue aplastado por una puerta hidráulica de una trituradora de metal. José estaba tratando de quitar un pedazo de metal atorado en la puerta hidráulica. La energía del sistema no había sido descargada y la puerta no había sido bloqueada para mantenerse abierta. José no había desconectado la máquina y no le había puesto un candado para que otro trabajador no pudiera encenderla. José cortó el pedazo de metal atorado con una antorcha. Cuando se desatoró el metal, se le cerró la puerta encima. La compañía no requería una confirmación visual de un supervisor para verificar que la máquina estuviera desconectada y con candado antes de que se le diera mantenimiento a la máquina. Recomendaciones: Los trabajadores deben: Asegurarse que las máquinas estén desconectadas, con candado y etiqueta de advertencia antes que cualquier persona les de mantenimiento. Los dos incidentes que aparecen a continuación se tratan de mecánicos de automóviles que murieron cuando los soportes de un gato hidráulico se movieron, causando que los vehículos o la maquinaria cayeran encima de ellos y los aplastaran. Un montacargas cae y mata a un mecánico: Un mecánico de 52 años, Pedro G.* (* No es el nombre real de la víctima), murió cuando un montacargas se deslizó de un gato hidráulico y el montacargas lo golpeó en la cabeza. Pedro alzó el montacargas usando el gato hidráulico. No usó soportes, torres, bloques de madera o ninguna otra ayuda rígida y estable para sostener el montacargas después de haberlo alzado con el gato hidráulico. El área del taller debajo del montacargas estaba un poco inclinada. Pedro colocó un bloque de madera detrás de sólo una de las llantas del montacargas. El montacargas se deslizó mientras él estaba acostado en la camilla para mecánicos debajo del montacargas. Algunos trabajadores piensan que pueden trabajar de una manera segura debajo de vehículos que están apoyados solamente en un gato hidráulico. El gato hidráulico nunca debe ser el único apoyo de un vehículo cuando alguien está debajo de ese vehículo. El gato debe ser utilizado para levantar el vehículo lo suficiente para poder colocar soporte adicional. Un autobús aplasta a un mecánico: Un mecánico de 48 años, Jeremy L.* (* No es el nombre real de la víctima), murió cuando un autobús que se cayó de los soportes de un gato hidráulico lo aplastó. Jeremy había levantado el autobús con soportes que sostenían cada llanta trasera. Después, puso soportes debajo de la suspensión trasera. Los soportes del gato no eran de un diseño común. Las llantas delanteras no habían sido bloqueadas. El autobús se deslizó de los soportes mientras Jeremy arreglaba los frenos. El autobús aplastó a Jeremy entre el eje trasero y el piso de cemento. Los vehículos pueden moverse si las llantas no han sido debidamente bloqueadas. Como mínimo, los bloques deben ser colocados en ambos lados de la llanta que está diagonalmente opuesta a donde se coloca el gato. Recomendaciones: Apoye los vehículos adecuadamente. The two young men were in the process of helping a silo dealer/distributor service representative conduct maintenance in the silo when the incident happened. There were no witnesses to the event, as the silo representative left the site temporarily during the time of the incident, and the other farm workers, including the farm owners, were working elsewhere on the farm site. The two victims were discovered unconscious inside the 90-foot silo shortly after the silo representative had returned to the farm. The farm owner, as soon as he learned that the two young workers were unresponsive in the silo, attempted to rescue them. Emergency medical persons were called and responded to the incident. Both of the victims died at the scene. Physical rescue and emergency response was hampered by having untrained persons attempting a confined space rescue at height, and the time-lag of the rescue personnels' arrival at the site. To prevent similar occurrences, the Washington State Fatality Assessment & Control Evaluation (FACE) Investigative team concluded that employers working on farms and/or other operations that work with silos and confined spaces should follow these recommendations: 1.) Employers should have a detailed confined space entry plan in place for all confined spaces. 2.) Employers should review and use alternative methods so that confined space entry is not required. 3.) Employers should consider contacting external expert consultants/contractors to help with confined space management and confined space entry processes. 4.) Employers and contractors need to follow manufacturers' recommended maintenance procedures. 5.) Employers need to have processes in place that prevent unauthorized entry. 6.) Employers need to maintain close supervision of all teen employees and contractors. 7.) All permit-required confined spaces must have detailed rescue processes and personnel in place. The driver was north bound on an interstate highway when witnesses saw the semi in the left lane contact the concrete median barrier then veer over to the right across one traffic lane and the shoulder, then strike a rock wall. The impact caused the cab to overturn onto the driver's door while the refrigerated trailer remained standing upright. An exhaust pipe located on the back of the cab was horizontal facing skyward on the cab. The fuel line to the refrigeration unit was damaged in the crash allowing fuel to drip onto the hot exhaust pipe and ignite a fire. Numerous calls reporting the crash were placed to 911 emergency medical services. A man and woman reportedly driving behind the semi witnessed the fire start between the cab and trailer. Emergency services arrived and found the cab and front of the trailer engulfed in flames. The local coroner was contacted and called to the scene where he declared the driver dead at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Semi tractor-trailer drivers should be trained to recognize signs of fatigue and when to seek appropriate rest areas. Recommendation No. 2: Highway median barriers should be designed with local input of emergency responder management allowing entrance of emergency responder personnel. Recommendation No. 3: Long-haul semi tractor-trailer drivers should have a network of mechanics along the routes they travel in case mechanical problems arise. Recommendation No. 4: Employers should require drivers to perform pre-trip and post-trip inspections on the tractor and trailer prior to operating them using an expanded Department of Transportation Inspection form. Recommendation No. 5: Manufacturers of commercial refrigerated trailers should consider a sensor design that shuts off the fuel supply to the refrigeration unit when the fuel line is compromised.
