Twenty-four-hour secretory patterns of gonadotropins and prolactin in a case of Chiari-Frommel Syndrome.

Description:A study was undertaken of the luteinizing hormone (LH), follicle stimulating hormone (FSH), and prolactin secretory pattern obtained by 20 minute interval plasma sampling for 24 hours in a 27 year old female patient with the Chiari-Frommel syndrome. The patient had presented for treatment of persistent postpartum amenorrhea and galactorrhea. Secretory patterns for LH, FSH, and prolactin during control and post clomiphene administration periods indicated that LH and FSH secretions were higher in the morning and afternoon than at night, particularly during the nocturnal sleep period. Plasma prolactin was higher during sleep than during waking hours in both studies. A particularly striking reciprocal relationship between prolactin and LH concentrations was noted. Clomiphene-citrate administration resulted in a comparable rise proportionately in LH secretion during the day and night while the absolute rise was greater during the day. While the difference between the 24 hour mean LH for the two studies was not statistically significant, the FSH mean value was significantly greater in the second study. The finding that hormonal concentrations began to fall in the evening before the onset of sleep suggests the importance of some factor other than sleep itself in their control. These findings lend support to the relevance of the dopaminergic control of LH and prolactin secretion in man. [Description provided by NIOSH]
