Description:The design of a survey plan and a survey form to determine the quantity, types, and usage of personal protective equipment for employee protection against certain physical and chemical agents is described. A survey master sheet is employed for each item of personal protective equipment for each agent. This form provides information in such areas as wearer and use acceptance of personal protective equipment, opinions of effectiveness, problems and deficiencies, and recommendations to improve types or methods. Extensive appendices provide the master list of agents, agent information sheets, respondent contact letter, sample survey form and instructions, and sample agent survey master sheets.
