  Clinical aspects, diagnosis, and hygienic aspects of occupational hand-arm vibration are discussed and a correction procedure to the present limits in the International Standards Organization (ISO) draft is proposed. Vascular consequences, nervous system consequences and musculoskeletal system consequences are considered under endogenous constitutional influences. Regarding endogenous- acquired influences, abnormalities of the cervical spine aggravating the clinical consequences of damage from vibration are considered. The influences of muscular tension, cold, work posture, and skill of the worker also play a part in the pathogenesis of vibration induced disorders. A standardized method of stimulation electromyography with simultaneous examination of the transmission rate in the faster motor and sensitive fibers for neurological diagnosis is mentioned. Hygienic aspects include measuring exposure and determining exposure limits. Based on studies involving transmissibility measurement it is concluded that the vibration energy transmitted in the human body represents one of the most important contributors to the adverse effects of vibration. Hand transmitted vibration limits are discussed. The present ISO draft is considered to prevent the accurate evaluation of vibration exposure situations that differ from typical conditions of operating mechanical tools. Thus, the use of hand driving point impedance data are suggested as a basis for making necessary corrections. Technical preventive measures for vibration disease are discussed.
