Description:The NACDD Healthy Communities Still ACHIEVEing Success update report is a compendium of information that showcases healthy community changes of NACDD funded Action Communities for Healthy, Innovation, and EnVironmental changE (ACHIEVE) communities in the areas of increasing physical activity and healthy eating, reducing or preventing tobacco use, chronic disease management, and leadership. As such, the document is a collection of successes that can be used to inform and plan efforts of local communities and states that wish to employ similar strategies. This updated collection represents the sustainability efforts and outcomes data of NACDD’s ACHIEVE communities for the last two years since ACHIEVE ended in September 2012.
