Description:LA Links is a combined data-to-care and patient navigation program. The intervention uses routinely collected HIV surveillance data, cross-referenced with other secondary data sources (e.g., vital records, incarceration records, and driver’s license records), to identify and contact PWH who are not in care (NIC).
