The State Heart Disease and Stroke Prevention Program is administered by the Centers for Disease Control and Prevention (CDC). Through categorical funding, the State Heart Disease and Stroke Prevention Program is designed to increase the leadership of state health departments in cardiovascular health promotion and cardiovascular disease prevention and control and to expand and direct efforts to establish a national CVH program." - p. 3

Sheree Marshall Williams, Ph.D., was the lead author and was assisted by ORC Macro and members of the Cardiovascular Health Branch's team for Program Services, Intervention, and Evaluation (PSIE). The logic model was developed by the PSIE with assistance from ORC Macro and Thomas Chapel and Bobby Milstein of CDC.
