Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The victim and two other employees were taking measurements before making saw cuts in order to install the next set of rafters. The victim had been working from both a 12-foot extension ladder and a 6-foot stepladder. The investigators determined that it was likely that he fell from the stepladder. When he fell from the ladder he struck his head on the concrete patio. He died at the scene. The investigators found that both ladders were set up properly and that they had no defects. He told the choker setter to signal to go ahead on the turn, though he was still out front, within 15-25 feet of the turn. As the turn moved, the end of a 37-foot log dug into a bank, and swung into the air and crashed down on the rigging slinger. So here are some ways we can prevent something like this from happening where we work: Get in the clear before any lines are moved. Follow-up on training and supervision for safe practices, even with experienced workers. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). "Can anyone describe what it means to be "in the clear"? Or the safest location when a turn moves?" He was holding onto handrails on the door and rear of the cab for support. The skidder was running in second gear at half throttle on nearly level ground. The operator and passenger were talking during the ride. When the passenger did not respond the operator stopped to look and found the victim lying in the road behind him who had been crushed by the right rear wheel. So here are some ways we can prevent something like this from happening where we work. Never ride as a passenger on a machine unless an appropriate seat and operator restraint system is available. Management or I should develop a formal training process for operators of all mobile machinery. Management or I need to evaluate operators regularly to make sure everyone is following safety regulations, and that workers are properly trained to safely operate equipment. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). "What mobile equipment on site are designed to have passengers?" "How can we improve equipment operator training?" Discuss a similar or other potential hazard. While the turn was heading uphill, a narrow 41-foot-long pole with a sharp end came sliding downhill toward the crew. The pole narrowly missed the rigging slinger. The choker setter was struck and impaled by the pole, and was carried about 27 feet downhill. So here are some ways we can prevent something like this from happening where we work. On hillsides, stay alert for loose objects. Make sure that landing area is large enough to keep logs from rolling or sliding back downhill. Make sure that workers on the landing can communicate with the rigging crew. Make sure inexperienced personnel are trained and supervised. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). "Are there times we see a situation but not communicate it to others who may be in danger?" "Has anyone worked where the landing area was poorly planned and may have caused an incident?" Express your commitment to training people on communication between ground and landing site workers and that inexperienced personnel will be closely supervised. NIOSH estimates 30-40% of burn hospitalizations are work-related, and are the most common cause of work-related hospitalizations for younger workers in the accommodation and food services industry. Describing and tracking work-related burn hospitalizations will help identify high-risk worker groups, as well as help inform and evaluate targeted prevention efforts. This report is the third annual report on occupational burns in Michigan; it covers the years 2011 and 2012. These are the key findings: Work-related burns were identified through multiple reporting sources: There were 1,777 work-related burns including three deaths in 2011. Because six individuals each had two separate burn incidents, 1,771 individuals were burned at work in 2011. There were 1,763 work-related burns including three deaths in 2012. Because seven individuals each had two unique burn incidents, 1,756 individuals were burned at work in 2012. The employer based survey system administered by the Bureau of Labor Statistics (BLS) estimated 600 work-related burns in 2011 and 890 in 2012. The BLS employer based system estimate of work-related burns was 42.1% (1,490 of 3,540) of the total number of work-related burns identified in 2011 and 2012 in Michigan's multi-source surveillance system. The most common type of medical encounter was an emergency department visit (84.3% in 2011 and 79.9% in 2012). Sixty-five percent of all burns were among male workers in both years of surveillance. In 2011 83.0% of all work-related burns were among Caucasians; in 2012 79.8% were among Caucasians. The most common part of the body burned were wrists and hands (34.6% in 2011 and 37.9% in 2012) and upper limbs except wrists and hands (18.4% in 2011 and 17.8% in 2012). Second degree (67.5% in 2011 and 62.6% in 2012) and thermal (69.9% in 2011 and 74.3% in 2012) burns were the most common types of work-related burns. Three industries - Accommodation and Food Services, Primary Metal Manufacturing and Healthcare and Social Assistance accounted for more than half of all work-related burn injuries in both years of surveillance, 51.9% and 56.1%, respectively. Workers' Compensation was the expected payer in 65.8% of the 1,542 cases in 2011 and in 61.2% of the 1,570 cases in 2012 for which there was a medical record. Payer source could not be determined for 13.6% of 2011 and 14.2% of 2012 medical records reviewed. The Michigan OSHA program completed inspections at 133 worksites identified by the surveillance system where individuals were burned in 2011 and 2012. MIOSHA issued 343 violations and assessed $784,440 in fines. The lumberyard laborer was walking from his work area to the employee lunchroom. At the same time, and in the same area, a coworker was operating a forklift that was loaded with lumber. The forklift operator's field of vision was limited because he was transporting the lumber "load-forward" and the load partially obscured his view. He did not see the laborer but stopped when he felt the forklift roll over something. He exited the cab and found the laborer unresponsive, lying near the left side of the forklift. The laborer was pronounced dead at the scene. The medical examiner identified head and thoracic injuries as the cause of death. The victim was wearing a dark gray jacket with orange reflective stripes on each sleeve, above each front pocket and across the upper half of the back, a white hard hat and dark trousers. He was holding a small flashlight and a non-reflective stop sign. As he was standing in the roadway, he was struck by a 2002 red Chevrolet S10 pickup truck in the right northbound lane, traveling at 50 mph. The driver of the pickup truck stated he did not see the flagger. According to the driver's operator license, the driver was required to wear corrective lenses. The driver was not wearing the corrective lenses at the time of the collision and informed the officer at the scene that he had a cataract affecting his right eye. Witnesses called Emergency Medical Services (EMS) and administered CPR. The EMS arrived and declared the victim dead at the scene. The victim did not have experience as a flagger and was not wearing highly reflective clothing as required by the Manual on Uniform Traffic Control Devices (MUTCD) code, which requires a class 3 high visibility reflective safety vest and a reflective stop sign for those flagging at night. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers should implement and enforce traffic control-related worker safety training in addition to properly setting up an MUTCD. Recommendation No. 2: Employers should provide proper MUTCD reflective clothing when employees are required to perform traffic control activities. Recommendation No. 3: Employers should avoid assigning traffic control activities after dark. Recommendation No.4: When corrective lenses are prescribed and an operator's license is restricted to driving with corrective lenses, motorists must wear the corrective lenses while driving. The day of the fatality, the victim and his coworker arrived to work at the farm where the incident later occurred. Their supervisor, the site manager, instructed them to clean the finishing barn's concrete floors and steel gate partitions using the electric-powered high pressure washer they had used in other buildings that week. At mid-morning, the coworker left to work at a different farm, but he returned to eat lunch with the victim over the noon hour. During lunch, the two workers discussed that the washer continued to malfunction just as it had during the week. After lunch, the coworker went back to the farm where he had been working, and the victim resumed pressure washing alone. The site manager returned to the barn at 15:30 to check on the victim's progress and saw him inside, slumped over a steel gate partition along the center aisle of the main room. The manager tried to rouse the unresponsive victim and pulled him to the loading dock area adjacent to the main room. The victim was cold and without a pulse. The manager called 911, and he then disconnected power at the circuit breaker and turned off the faucet supplying water to the pressure washer. Within several minutes, the county Sheriff, an ambulance crew, and local emergency responders arrived from a town nearby. The county Medical Examiner arrived at the site and pronounced the victim dead at 16:00. The position of the victim's body when found suggested that he had been leaning over the gate to reach the pressure washer. An autopsy identified an electrical burn injury to the victim's right thigh where it had contacted the steel gate. The cause of death was determined to be electrocution. Law enforcement agencies and the Iowa Division of Labor Services/Occupational Safety and Health Bureau (IOSHA) investigated the fatality. Interviews with the victim's manager and coworker revealed that the pressure washer had a history of electric malfunction that was known to the manager and employees for several days preceding the fatality: workers who handled the metal wand or high pressure hose had received electric shocks AB - the machine "threw sparks" when its metal wand contacted other metal surfaces in the buildings AB - power surges of the pressure washer motor caused room lights on the same circuit to dim AB - and the machine periodically shut off while in operation. Four days before the fatality, it had short-circuited, tripping circuit breakers and burning a receptacle at a different farm building. The pressure washer was taken to a local dealership for inspection by the manufacturer's product engineers and the IOSHA compliance officer. They found evidence of water in the metal electrical panel box that had caused extensive corrosion. Inside the electrical panel box, there was thermal damage to conductor insulation and electrical components caused by current overload. Damage to conductor insulation created a situation wherein 1) the exposed hot and neutral conductors could contact each other, creating a short circuit, and 2) the exposed conductors could contact the interior surface of the electrical panel box, creating a fault that energized the panel box and all connected metal parts of the pressure washer, including the frame, wand, and exposed braiding on the high pressure hose. The pressure washer model involved in this incident was not certified to the UL 1776 voluntary pressure washer safety standard which specifies a number of safety features designed to protect the operator, including built-in ground fault circuit interrupter 2 (GFCI) protection. The pressure washer and the building installation both lacked GFCI devices designed to open the circuit upon detection of low levels of current flowing along an unintended path. When the victim touched a metal surface of the energized pressure washer at the same time his body was in contact with the steel gate, his body became part of the fault current path to ground. Factors contributing to this incident include: 1. Use of equipment with a known history of electrical malfunction that was not maintained in safe operating condition. 2. Use of electrical equipment without GFCI protection. 3. Workers were not provided training in operation and safety hazards of electrical equipment. To prevent future incidents of this nature Iowa FACE recommends that employers: 1. Use pressure washers only in applications where ground fault circuit interrupter (GFCI) protection is present and in working order. 2. Maintain pressure washers in proper operating condition AB - remove malfunctioning equipment from service until repairs have been made by qualified repair personnel. 3. Train workers on electrical hazards associated with operating pressure washers, including risks for electric shock, routine inspection and maintenance, and proper use of equipment AB - verbal training and written materials should be provided in workers' primary language. The worker was a member of a contract cleaning crew for a meat processing facility. On the day of the incident he reported to work and began routine cleaning and sanitation. Cleaning procedures began with a hot water wash of the rotating blades and mechanical parts to remove residual meat product. The hoses used in rinsing were long, and it was a common practice for workers to wrap the excess hose around their bodies (torso and/or legs). The incident was not witnessed, but based on the cleaning process used by the worker and described by other employees, it is believed either the hose used to wash down the operating equipment fell into the hopper and the worker was pulled in while entangled in the hose or that he lost his footing on the slippery platform and fell into the blender vat. The worker's supervisor who was familiar with the sounds of the machinery, investigated the source of an unusual sound and discovered a severed hose. He then climbed the stairway onto the elevated platform above the blender and saw the worker and additional hose entangled in the blades in the blender vat. The supervisor summoned help and emergency responders were called. The worker was pronounced dead at the scene. RECOMMENDATIONS: 1. Host employers should remove or reduce worker exposures to moving machine parts by identifying and installing guards or interlock features. 2. Host employers should establish routine procedures for communicating and reviewing hazards in their work environment and control methods with contractors. 3. Where tasks such as maintenance or sanitation require removal of safety guards, host and contract management must collaborate to ensure that lockout/tagout procedures are implemented including employee training, providing locks for employees, and conducting periodic workplace observation. 4. Contract/temporary employers should assess tasks proposed by the host employer and collaborate with the host employer to clearly outline supervision responsibilities, training requirements and hazard identification and control methods. 5. Contract/temporary employees should be trained on injury reporting procedures and hazard recognition and control. Once trained, they should be encouraged and provided positive feedback for identifying hazards, recommending controls, engaging in best safety practices and stopping hazardous work practices. Oregon Institute of Occupational Health Sciences AB - It was the end of the day, workers were returning to the equipment shop after applying pesticide in the vineyard. On the crew was a 41-year old worker who had worked for the employer for five years. Witnesses reported that he was standing on a make-shift wooden platform that had been attached to a trailer mounted 400 gallon chemical tank containing pesticide. The make-shift wooden platforms were created and installed by the employer. The trailer was in tow by a tractor. The worker's baseball cap blew off while he was riding on the platform on the moving trailer. He stepped off the platform to retrieve his cap. Once back on the platform, or as he was stepping onto the platform, the worker lost his balance and fell to the ground. He sustained a fatal head injury in the fall when his head struck a large rock. RECOMMENDATIONS 1. Employers should consult the manufacturer or a professional engineer before modifying, installing or retrofitting components of equipment. 2. Employees should be trained on hazard recognition and control. Once trained they should be encouraged and reinforced for reporting unsafe conditions or practices. 3. Training on the use of personal protective equipment (PPE) should include not only proper use, but also limitations including how ergonomics and mobility may be affected during use. The 73-year-old roofer landed first and the second roofer landed on top of him. Both men fell 15 feet and 6 inches and landed on a sloped grassy area. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers should not allow their roofers to access roofing areas in times of windy, wet, or icy or conditions to prevent falls. Recommendation No. 2: A roofer should use a personal fall protection system during all phases of a roofing job. Recommendation No. 3: Employers should have a written fall protection program in place. The weather was overcast, and it had rained the day before, when the vendor (two co-owners and two workers [one was the victim]) arrived at the worksite. The victim was shimmying down a grain spout holding a sprayer hose to paint a grain bin when he lost his grip and fell. He was not wearing any type of fall protection. One of the vendor co-owners heard a noise, went to investigate and found the victim lying on the concrete, unresponsive. The co-owner immediately dialed 911 at 4:00 pm. Emergency medical services arrived on the scene at 4:16 pm and assessed the victim as unresponsive with rapid shallow breathing. At 4:19 pm the victim was transported by helicopter to a local trauma center in the next state and pronounced dead at 7:35 pm. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Regardless of company size, the employer must furnish proper personal protective equipment to employees conducting hazardous work. Recommendation No. 2: Employers must train employees on how to properly wear the personal protective equipment provided as well as provide safety training for employees. He did not lockout, tag-out, and verify hazardous energy shut down before servicing the saw. The victim reached into the energized electrical panel box to reset a relay and came into contact with 480 volts of electricity. This shock was powerful enough to send him into cardiac arrest. The employees who were working on the saw immediately turned around and discovered the victim lying unconscious on the ground. One employee called 911 at 11:45 am, while the other employee performed CPR. EMS arrived at 11:56 am and the victim was transported to the local hospital, where he was pronounced dead. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers should ensure that all employees servicing or maintaining electrical equipment practice proper lockout/tag-out (LOTO) procedures. The victim was using a backhoe's loader to scoop up the rocks and carry them from the field. As he was doing this task, he noticed a metal chisel which had broken off the plow lying on the ground. He stopped the backhoe and set its brakes, but did not turn off the engine. He got out of the cab and walked over to the curved three-foot long, 50 lb. chisel and picked it up. He then walked to the rear of the backhoe and while standing between the raised boom and the raised stabilizer, he placed the chisel in the cab. The chisel struck the right boom-swing foot pedal which activated the boom causing it to move, pinning and crushing him between the boom and the stabilizer. The goal of this awareness material is to: Help workers to recognize what a confined space is, to identify the associated hazards and to clearly understand that they should NEVER enter a confined space unless they have received appropriate training and the entry is in accordance with the employer's written confined space entry program. Assist employees in understanding the correct emergency response in a confined space incident. A confined space workplace emergency situation may generate spontaneous reactions that may lead to multiple fatalities. It is critical that workers know they should NEVER enter a confined space to rescue someone unless they are qualified Confined Space Rescue personnel. CALL 911 immediately! Don't waste time. During the tailgate discussion, you will be providing your workers with information to identify confined spaces as well as the hazards that can be present in confined spaces. The victim was standing on the elevated catwalk between rows of plating tanks and was most likely leaning over a tank checking on a recent pump installation when the traveling hoist rolled past him, pinning him against the tank. The incident was not witnessed. The hoist operator stated that he was not aware that the victim was in a restricted area (danger zone) between the tanks. The traveling hoist was computer-controlled and moved automatically, and was programmed and monitored by an operator located at the front end of the plating tanks. The safety sensor device on the traveling hoist was designed to immediately shut off when it made contact with an object. However, this device was non-operational at the time of the incident. The CA/FACE investigator determined that in order to prevent future incidents, employers with moving machinery within a restricted work areas should: 1. Ensure that moving machinery is inoperable when employee access into a danger zone is required. 2. Ensure that emergency safety devices are maintained in operating condition. Then read the story. Our safety talk today is about a 62-year-old truck driver who died after being crushed between his semi-trailer and a loading dock. After the delivery, the driver pulled forward and remembered that he had left a tie-down strap in the warehouse. The truck was placed in neutral and a "trailer brake" was set. The wheels of the trailer were not chocked and the tractor parking brake was not set. This caused the truck and trailer to move crushing the victim against the loading dock. So here are some ways we can prevent something like this from happening where we work. Make sure that parking brakes are set before leaving the cab or motorized equipment. Use wheel chocks to prevent unexpected movement of tractors and/or trailers, especially when parked on a slope. Make sure that you do not stand in pinch points where vehicles could move or roll over. Management or I should try to rent or purchase equipment with built in protective features during pre-planning. Management or I need to make sure everyone is trained to operate rental equipment prior to use. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). "Are there any operations we do that might cause us to leave a vehicle or motorized equipment without setting the brake? "What do you do when you see someone working or standing in a pinch point?" Discuss a similar situation at your current site. Express your commitment to training people to identify pinch points. Commit to follow-up at the next safety talk.
