Toolbox Talk Guide: Truck Driver Crushed Between Semi-Trailer and Loading Dock

Our safety talk today is about a 62-year-old truck driver who died after being crushed between his semi-trailer and a loading dock. After the delivery, the driver pulled forward and remembered that he had left a tie-down strap in the warehouse. The truck was placed in neutral and a "trailer brake" was set. The wheels of the trailer were not chocked and the tractor parking brake was not set. This caused the truck and trailer to move crushing the victim against the loading dock. So here are some ways we can prevent something like this from happening where we work. Make sure that parking brakes are set before leaving the cab or motorized equipment. Use wheel chocks to prevent unexpected movement of tractors and/or trailers, especially when parked on a slope. Make sure that you do not stand in pinch points where vehicles could move or roll over. Management or I should try to rent or purchase equipment with built in protective features during pre-planning. Management or I need to make sure everyone is trained to operate rental equipment prior to use. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). "Are there any operations we do that might cause us to leave a vehicle or motorized equipment without setting the brake? "What do you do when you see someone working or standing in a pinch point?" Discuss a similar situation at your current site. Express your commitment to training people to identify pinch points. Commit to follow-up at the next safety talk. Description provided by NIOSH
