Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

A national FACE program was begun by the National Institute for Occupational Safety and Health (NIOSH) in 1982, and was expanded to the states in 1992. Oregon joined 14 other state FACE programs in October 2002. According to its mission, OR-FACE has developed staff and technical capacity for surveillance, investigation, assessment, and outreach related to occupational fatalities in the state. A team of four contribute management, data and outreach, investigation, and analysis, respectively and together. The 76 occupational fatalities in Oregon in 2003 correspond to an incident rate of 4.45 per 100,000, when counting all workers in the civilian labor force. Further use of rates for specific occupations is avoided in this report, due to common confusion in the use of employment statistics. The civilian labor force includes both employed and unemployed, but some reported rates use only the employed, many exclude the self-employed, while others adjust for military personnel. Rates are also less reliable with only 1 year of data. With descriptive detail, this report provides a snapshot and a foundation for later analyses. Of immediate value, the story of each loss of life presents a lesson in itself. WHAT WENT WRONG? The work was done directly under an unprotected, energized, high-voltage line. David and Shawn had not been trained how to safely use a crane. WHAT SHOULD BE DONE? Don't work near energized high-voltage lines unless necessary. If work has to be done near power lines, make sure that there is a plan to do the work safely, that the plan is followed, and that the workers have the right equipment to keep themselves protected. Make sure workers using cranes are fully trained and follow safe work practices. The victim was removing carpet glue from a concrete floor in the basement "den" area of a townhouse unit with a flammable lacquer thinner. See Figure 1 for an example of a townhouse basement unit layout. The victim was assisting a co-worker in preparing the townhouse unit for a new tenant. While her co-worker was cleaning the second story of the townhouse, she proceeded to the basement. The mechanical room contained the gas hot water heater and furnace, plumbing and electrical for a washer and dryer, and a laundry tub. The victim was using the flammable lacquer thinner on the floor to soften the carpet glue so she could remove it. Although both basement rooms had 17" x 11" windows, she did not open them. It is hypothesized that she poured the lacquer thinner on the floor. The vapors from the lacquer thinner migrated into the mechanical room, contacted the open flame of the hot water heater and flashed back and burned her. Her co-worker working upstairs heard the victim scream and came to assist her. The co-worker found the victim at the top of the basement stairs and assisted her outside. The co-worker used her walkie-talkie to contact the housing commission office, and office personnel contacted 911. The victim was transported to a local hospital where she died approximately a week later. Recommendations: 1. Employers should develop and implement a hazard communication (Right-to-Know) program. 2. Employers should investigate if substitution of water-based, non-chlorinated cleaning products in place of flammable materials used for cleaning is possible. 3. Employers should develop and implement a site-specific health and safety program. 4. Michigan Housing Directors Association (MHDA) should establish a Health and Safety (H&S) committee, as should each individual housing commission. 5. Housing commission units should consider replacing older hot water heaters with heaters that are equipped with flammable vapor ignition resistant (FVIR) components. 6. Gas hot water heater manufacturers should place fire hazard warning labels near the pilot light as well as in another observable location on the unit. They were climbing the stepladders while holding the wooden beam with one hand and resting it on their shoulders. The beam was to be set on posts that were about 11 feet, 9 inches tall and spaced about 15 feet apart. The victim was about to place his end of the beam on the post when he fell from the stepladder. His head struck the concrete patio below and he subsequently died of head injuries from the fall. An investigation determined that the stepladders used by the contractor employees, were overloaded beyond the manufacturer"s rated capacity, and that the two employees were standing on the top steps of the ladders. Fatality Assessment and Control Evaluation (FACE) Program AB - On May 17, 2004, a 36-year-old Hispanic laborer (the victim) died after he entered a sawdust storage silo to unclog a sweep auger advance mechanism and became engulfed in sawdust. Several minutes after the victim entered the silo, a Hispanic coworker working nearby heard a noise from the silo which prompted him to call out to the victim. When he heard no response, he and several Hispanic co-workers began looking for the victim. After approximately 20 minutes of digging through the collapsed sawdust, they found him. Coworkers ran to several locations trying to report the incident and get help. They notified a company senior official who then called 911. Emergency Medical Services (EMS) responded within approximately 30-35 minutes of the presumed time of engulfment and attempted resuscitation. They transported the victim by ambulance to a local hospital where he was pronounced dead by the emergency room physician. The equipment that was being used to lift the load was a rough terrain forklift. The load being lifted was extended beyond the ability of the rough terrain type forklift to counterbalance and it upended. The load shifted and the falling bundle of lumber struck the victim, who was attempting to climb through a stairwell hole to access the second floor at the same time. The victim was killed instantly. Neither the loader operator nor the victim was aware of the others position or activity. Recommendations: 1. Never work under a load being lifted into position. No one should be allowed within the swing of a crane or lift, unless they are expected to be there e.g., spotters, guides etc. No one should ever be allowed to be directly under the load at any time. 2. Stay within the safe working parameters of the crane or lift and do not exceed the recommended load or reach specifications. Ensure that all mobile equipment operators are trained or certified in the operation of the equipment prior to use. 3. Designate a person to act as a spotter and communicate with others on site and the equipment operator. There should be a communication system worked out between all workers, i.e., hand signals, whistles, walkie-talkies (if out of visual range). 4. Communicate to workers what kinds of work activities are planned and any safety expectations. 5. As he worked from the aerial lift suspended over traffic to change the burned-out bulb, a cargo truck drove underneath the bucket, striking it, and causing the decedent to fall to the asphalt below. As the worker fell approximately 10 feet, an eyewitness called emergency services. Paramedics arrived within minutes and transported the technician to a nearby hospital. From there, the technician was airlifted to a trauma level hospital where he died 4 days later due to head injuries. To prevent future occurrences of similar incidents, the following recommendations have been made: 1. Employees should use personal fall protection equipment in an aerial lift bucket at all times. 2. A Competent person should conduct a hazard assessment of the highway work zone and implement a safety plan. After driving the golf cart into the barn, the farm owner placed two square bales of hay on the golf cart: one on the back and one on the passenger side of the front seat. Leaving the ignition key of the golf cart in the "on" position, she left the golf cart in the aisle at the end of the barn. She then walked the length of the barn, entered a stall, mucked it out, exited the stall into the barn aisle, and leaned the pitchfork against the wall next to the stall door. Unbeknownst to the farm owner, the bale of hay on the passenger seat had slipped off the seat and fallen onto the gas pedal of the golf cart. With the pedal depressed by the hay bale and the key in the "on" position, the golf cart proceeded down the barn aisle, striking the victim and pinning her underneath. Several hours later, the farm owner's daughter went to the barn and found her mother pinned underneath the golf cart. Upon finding her mother, she called a friend then went to the farm manager's house located nearby on the farm. The daughter informed the farm manager that her mother was in the barn pinned underneath the golf cart. The farm owner's daughter returned to the barn with the farm manager to find the daughter's friend and her friend's mother on the phone with emergency services. Emergency services arrived and contacted the coroner who arrived and declared the horse farm owner dead at the scene due to "compression asphyxia with hypothermia". To prevent future occurrences of similar incidents, the following recommendations have been made: 1. When exiting a golf cart, the operator should always turn the ignition switch to the "off" position and remove the key. 2. Golf cart seats should only be used for human occupancy. 3. A golf cart should be equipped with appropriate accessory equipment specific for the task. The objective of FACE is to prevent and reduce the severity of workplace injuries by identifying high-risk work situations, developing prevention strategies, and informing those who can intervene in the workplace. The FACE program uses multiple sources of notification for the identification of fatal occupational traumatic injuries. FACE investigations target fatalities involving machinery, street/highway construction work zones, youth (under 18), and Hispanic workers. Recommendations for prevention are included in every investigation and disseminated to employers and workers nationwide. There has been a significant downward trend in both occupational fatality and homicide rates in Los Angeles County for the period 1992-2002, but this has not been the case for other causes of death (transportation, machine-related, and falls). The fatality rate for Hispanic workers was approximately 50% greater than it was for non-Hispanic workers (39 vs. 26 per 100,000 workers). Thirty-seven percent of all fatalities were homicide, followed by transportation (18%) and falls (12%). Among homicides, 45% were robberies, and 88% involved a firearm. Homicide was the leading cause of death for both male (36%) and female (51%) workers, and accounted for 87% of supervisors of sales occupations, 80% of security guard and 91% of cashier fatalities. Homicides have been the leading cause of occupational fatalities for each year 1992-2002. Although overall fatality and homicide rates have decreased, deaths from other causes have not. Hispanic workers are at increased risk of dying on the job. The FACE program identifies risk factors in investigated cases. Identify and label equipment that is not to be operated by workers less than 18 years old. 4. Ensure that employee orientation and safety training is given to employees before they begin work. The CA/FACE investigator determined that, in order to prevent future occurrences, employers, as part of their Injury and Illness Prevention Program (IIPP), should: 1. Ensure employees do not reach into the hopper of trash trucks when they are cycling. 2. Ensure employees that are operating the trash truck compaction controls constantly monitor the compaction process. These incidents are preventable by assuring that proper procedures are followed. How to Prevent Truck Loading and Unloading Incidents: 1. All personnel on foot must be clear of the loading zone during operations. 2. Properly secure loads on trucks, forklifts and cranes. 3. Loaders & drivers must know and practice hazard ID and safe work practices. 4. Institute procedures to assure people are clear of trailers/wheels before moving. Additional Controls for Prevention: 1. Protect all gaps and drop-offs at loading docks 2. Assure appropriate forklift equipment and training 3. Never be downhill of a moving load 4. The victim was working near a barn and used a tractor and a power-take-off (PTO) driven forage wagon to deliver forage to cows in a barn. The self-unloading wagon was equipped with two unloading beaters mounted across the front of it. The beaters broke apart the forage in the wagon as two chain conveyors on the floor of the wagon moved the forage to the front of the wagon. At the time of the incident, the forage wagon was nearly empty. One of the conveyor chains on the floor of the wagon had broken and as a result, the forage would not move forward toward the unloading beaters. As a result, the victim entered the forage wagon to shovel the contents from the wagon while the unloading beaters continued to operate. The victim's step-father was working inside the barn and heard the victim scream. He ran outside and discovered the victim caught in the beaters. He stopped the tractor's PTO drive and then ran to the farmhouse and placed a 911 call. He returned to the victim who told him that he leaned over the top beater to determine if he needed to shovel any more of the forage from the wagon. When he leaned over the top beater bar, his jacket became caught in one of the tines of the top beater. Emergency personnel arrived at the scene shortly after being called and assisted in freeing and removing the victim. After the victim was freed, he was placed in an air ambulance helicopter and airlifted to a major medical facility. After he arrived at the medical facility and was admitted, he died several hours later from the injuries he received during the incident. MN FACE investigators concluded that, in order to reduce the likelihood of similar occurrences, the following guidelines should be followed: 1. Operators should, whenever possible disengage the power-take-off before dismounting from a tractor. 2. Working youth should only be assigned age appropriate tasks, and AB - 3. He had attached a New Holland chopper to the rear of the Farmall 400 tractor and a forage wagon to the chopper to hold the cut hay. It is unknown how he came into the position of being run over by the tractor's rear wheel and the chopper. Several scenarios have been hypothesized. The victim was on his third pass in the middle of the field. After the victim was run over by the equipment, the tractor continued to travel forward in the field in an arc from east to west, finally coming to rest in a tree line adjacent to the hay field. When the victim did not return home family members went to search for him. They found him in the field and called for emergency response. Emergency Response declared the victim dead at the scene. Family members noted that all prior forage cuts the victim made were straight, except in the location just before where the victim was found. Family members noticed a very slight "zigzag", from west to east in this final cut. The recommendations below are based upon the possible event scenarios described in the report. 1. Ensure medical conditions are managed by all workers on the farm. 2. Place cell phones on your body where they won't fall off or interfere with work activities. 3. Stay seated on the tractor seat while operating the tractor. 4. Do not try to retrieve items that fall from the tractor while the tractor is moving. 5. Make sure you are rested when operating machinery - establish a work routine that includes adequate rest breaks and limits on continuous hours of work. 6. Start tractor from the operator's seat. 7. El sujeto había trabajado como empleado por tres días para una compañía de paisajismo contratada para despejar el follaje localizado sobre el lugar de paso de una línea de gas. A las 2:20 p.m. aproximadamente, el propietario de la compañía y cinco trabajadores se encontraban derribando árboles y cortando los troncos derribados haciendo uso de sierras eléctricas y cortadoras con el fin de remover los árboles y las ramas que sobresalían y crecían sobre el lugar de paso de la línea de gas. Dos de los trabajadores se encontraban laborando en el punto más alto del extremo izquierdo del lugar de paso de la línea sobre la colina; el occiso y otro trabajador se encontraban laborando sobre el borde derecho hacia la mitad de la colina; el propietario de la compañía y otro trabajador estaban localizados sobre el borde izquierdo del paso de la línea en la parte inferior de la montaña. El equipo de la cumbre derribó un árbol de nogal en dirección del terreno en cuestión. A continuación uno de dichos trabajadores cortó el tronco derribado en trozos de aproximadamente 30 pulgadas de longitud. Uno de estos troncos de 30 pulgadas rebotó y rodó colina abajo una distancia de 200 a 300 pies, e impactó a la víctima en la cabeza y el pecho. A coworker dumping a load of rock onto the conveyor of the crusher noticed the victim missing from his normal position and spotted him slumped in the corner of the lower platform. The victim was bleeding profusely from the neck, with the pry bar in his lap. Efforts to stop the bleeding were unsuccessful. The victim died at the scene. Recommendations: 1. Never attempt to remove jammed material while machinery is in operation. Shut down, isolate, and block all hazardous energy before performing machine maintenance. 2. Do not allow employees to position themselves directly over the intake of a horizontal impactor while it is actively crushing rock. 3. Manufacturers should consider designing and installing viewing decks where workers can safely access the equipment to observe operation.
