Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

On the day of the incident, the victim was assigned by his dispatcher to pick up freight consisting of a full trailer load of chilled boxed beef at a customer's meat processing facility. The victim drove his truck to the customer location. He then drove to the empty trailer lot portion of the yard where he unhooked and dropped his refrigeration trailer. An employee of the facility hooked the trailer to a yard hostler and hauled it to the loading dock to be loaded with freight. After the trailer was loaded, the hostler operator deposited the trailer in the loaded trailer lot. Located across from the loading dock, the lot's surface is a mixture of asphalt and concrete with yellow lines marking the parking stalls. There are often several trailers parked in the lot. It is a part of the yard where loaded trailers are left awaiting their drivers who then back their trucks up to their trailers and hook them up and prepare for departure. Drivers may pick-up their trailers at any time during the day or night. The victim backed his truck up to his trailer and hooked it up. As he was doing this and preparing to depart, he chatted with the driver of a truck in the adjacent parking stall who was also preparing to depart. The driver of the other truck finished his pre-trip check and got into the cab of his truck. He reported that he last saw the victim standing at the rear of his own truck's trailer. Less than a minute after last seeing the victim the other driver checked his mirrors and started moving the truck forward out of the stall. As he drove forward he made a sharp turn to the left in order to avoid a hostler parked in front of his truck. The victim was standing on the driver's side of his trailer near the front cranking up his trailer's landing gear. It was dark and he was not wearing the high-visibility safety vest which his employer had issued him. As the other truck moved forward and swung left the right rear of its trailer struck the victim. The driver was not aware that the victim had been struck and proceeded to leave the yard. The victim died at the scene. Recommendations: To prevent similar occurrences in the future, Washington State Fatality Assessment and Control Evaluation (FACE) investigators concluded that to protect employees from the hazard of being struck by a moving truck while working outside of their truck in parking, and freight loading/unloading areas employers and facility owners should follow these guidelines: 1. Perform a site survey of facilities with truck loading and unloading areas, parking areas, and yards where there is truck traffic to identify potential truck vs. pedestrian hazards and take corrective actions to prevent pedestrians being struck by trucks. 2. Design site layout of facilities and internal truck traffic routes to provide adequate space for trucks to safely maneuver, load, unload, and park. Facility design should also provide for safe passage for pedestrians. The level of lighting should be sufficient for safe vehicle and pedestrian activity. 3. Host facilities should create and enforce a policy requiring drivers to wear a high-visibility safety vest when outside their vehicle. 4. Train drivers to initiate and maintain communication with other drivers when working outside their truck in parking, loading, and unloading areas so as to ensure that their activities do not put them at risk of being struck by the movement of other trucks. 5. Provide truck drivers with appropriate high-visibility safety vests and train them to ensure that they are worn in an effective manner whenever they exit their vehicle. 6. Install appropriate mirrors on all fleet trucks to ensure that drivers have superior visibility and that blind spots on the right side of the vehicle are eliminated. 7. Truck manufacturers, equipment designers, and researchers should continue to develop onboard systems and equipment that allow truck drivers to better detect pedestrians. The owners of the farm and a family member were in the front of the barn AB - one of the owners was tending to a sick cow and the other owner and family member were in the break area when they heard the cows bellowing. The three of them stepped out into the feed aisle and saw the bull in the feed aisle walking toward them. The bull was herded without incident to another stall. One of the owners found the worker lying face down and unresponsive under a stanchion AB - with the upper body found in one stall and lower body in another stall. The owner rolled the worker over and began CPR while emergency response was summoned. The worker was declared dead at the scene. Contributing factors: 1. Bull freely roaming with dairy cows. 2. Worker entering stall to encourage reclining cows to stand and exit stall. 3. Possible underestimation of bull and cow aggressive behaviors. 4. Working alone with bull in dairy cow freestall. 5. Cubicle design/freestall design may not have had room for cow movement and employee escape. 6. Possible employee complacency when working around the animals and/or was distracted by other animal behaviors. RECOMMENDATIONS: 1. Establish worker rules/procedures and acceptable work practices for anyone (employee & family) working around dairy animals specific to the farm operation. One work practice should be that workers prompt animal movement from the resting position in a partitioned freestall from an empty freestall next to the animal being directed. 2. Train workers/family members to recognize animal health issues and attack scenarios and behaviors. 3. Ensure the freestall design permits cows to easily stand up when lying down in the stall to avoid animal handling & comfort issues. 4. Workers should not be permitted to be in a pen with a bull alone and should maintain constant awareness of the location and behavior of bulls when entering a holding pen. 5. Consider using artificial insemination, not a natural service bull, to impregnate cows. The forklift operator was unloading the slabs with a forklift equipped with a boom and a clamp designed to move the granite. The owner (victim) walked by the forklift to the area where the granite was to be stored. The forklift operator gently set the granite next to the victim who was standing directly in the middle of the large piece of granite guiding it into place (within the fall shadow zone). Once the granite was on the ground, the clamp automatically released. The granite shifted, striking the victim in the head and pinning him under the 1000 lb. granite slab. At the time of the incident, the wind speed was 15.0 mph and a gust of wind registered at 21.9 mph from the east. Weather could have been a factor in this fatality. The forklift driver, delivery driver, and another employee tried to remove the granite from the victim AB - EMS was immediately called and arrived within minutes. The victim was declared dead at the scene. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers should identify a fall shadow zone, and restrict employees from placing themselves in that zone, while stone is being loaded, unloaded or moved. Recommendation No. 2: Employers should provide written safety programs and training for their employees. Recommendation No. 3: Employers should provide hazard recognition training for their employees. Recommendation No. 4: Employers should always use stone slab storage racks designed by engineers with individual compartments to support the granite and marble slabs. Recommendation No. 5: Spotters should remain in full view of equipment operators at all times while wearing personal protective equipment. Recommendation No. 6: A guideline or regulation for slab transfer during windy environmental conditions should be established. Kentucky Injury Prevention and Research Center AB - July 16, 2014, a 34-year-old temporary electrician helper was laying out light fixtures to be installed in a commercial construction project when he stepped into an unguarded elevator shaft and fell 13 feet and 8 inches to his death. At approximately 5:00 pm, two sub-contracted painters were looking for empty buckets and found the victim at the bottom of the elevator shaft. It was unclear how long he had been there. Emergency Medical Services were notified and arrived on the scene, pronouncing the victim dead at 7:20 pm. His death was due to injuries sustained in the fall. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers should guard all open areas where falls could occur. Recommendation No. 3: Employers should ensure that all working areas are well lit. Recommendation No. 2: Employees should complete a minimum of 10 hours of safety training when hired. Recommendation No. 4: Employers should develop and implement drug free workplace policies and procedures to ensure a drug free workplace. Recommendation No. 5: Employers should hire experienced and qualified workers. Occupational and Environmental Medicine AB - In fall 2013, a male dump truck driver in his 30s died when his clothing became entangled on his Ford LTL 9000 dump box rotating power take off (PTO) shaft driveline. The decedent was subcontracted to drop off a load of gravel at the incident location and then meet the contractor at another location to pick up some concrete. The decedent arrived at the dump site with his gravel-loaded truck and attempted to raise the dump box. Apparently, the dump box would not lift up. After looking under the hood, with the truck running and PTO driveshaft rotating, the decedent went under the truck to try to ascertain the problem. A possible scenario was that he was attempting to determine if there were loose cable connections (used to raise and lower the dump box) at the "gear box" controlling the hydraulic valves. His clothing contacted the spinning drive shaft and wrapped around it, pulling his neck up against the driveline, choking him. The individual who subcontracted his services arrived at the incident site and saw the truck. He walked over, called the decedent's name, and tapped on the truck which had stalled. When the decedent did not answer, he looked under the truck. He immediately called 911. When emergency responders arrived, they cut the decedent's clothing free and pulled him from under the truck. He was declared dead at the scene. CONTRIBUTING FACTORS: The following factors have been identified which may have contributed to this incident: 1. Truck not turned off and PTO rotating when he went underneath the truck. 2. PTO driveline not shielded. RECOMMENDATIONS: 1. Truck drivers should be aware of the potential dangers associated with PTOs and prior to engaging in any work process that exposes them to a PTO, should ensure that the PTO is disengaged and the truck engine is shut off. 2. Dump truck drivers should perform a daily pre-trip inspection that includes raising and lowering the dump box, to ensure the truck is working correctly prior to driving and/or loading. 3. When working around rotating shafts, do not wear loose clothing. 4. Engineering changes should be developed to include a PTO driveline guard for existing machinery - including dump trucks - to shield any open, unguarded power take-off drivelines and locate the gearbox in a more accessible location. Occupational and Environmental Medicine AB - In spring 2014, a male lathe operator in his 20s died when his shirt became entangled on a 1 7/8-inch piece of 1-inch diameter bar stock. The decedent purchased the South Bend manufactured 16 inch lathe at an auction and placed the lathe in a dimly lit and congested area in the workshop near a shelving unit on a wall. The lathe had a pass through chuck. The work piece, a 1-inch round by approximately 3-foot long bar steel stock, had been cut to length with a band saw. A 1-7/8 inch piece of the bar stock extended from the end of the lathe (tailstock) AB - its edge was sharp and had a burr. The bar stock was located approximately 46 ½ inches above floor level. It was not known why the decedent was at the tailstock of the lathe. Several possible incident scenarios have been developed: 1) he was leaning over the rotating tailstock to obtain a tool from the shelves behind the lathe or 2) he walked behind the rotating tailstock to obtain a tool from the shelves behind the lathe. At some point, the right pocket of his shirt was caught by the rotating tailstock burr. His shirt wrapped around the stock and tightened at the decedent's neck and chest. A coworker working nearby heard a noise, looked over, and saw him. He ran outside and informed the decedent's father. His father instructed him to shut off the generator which powered the unit and then they worked to remove the decedent from the entanglement. Emergency response was called and the decedent was transported to a local hospital, where he died approximately two weeks later from brain injury due to a loss of oxygen. Contributing factors: 1. Walking to and/or reaching over unguarded rotating bar stock. 2. Edge of bar stock had a burr that caught on the decedent's clothing. 3. Position of lathe in congested area limited worker movement. 4. Decedent's level of training in lathe operation. RECOMMENDATIONS: 1.Before using a lathe, operators should receive appropriate training on proper lathe operation and the hazards associated with lathe operation. 2. Incorporate safe work practices when using a metal lathe, including appropriate positioning to allow safe operator movement and ensuring rotating shafts have come to rest when reaching over them. 3. Control exposure to hazards by use of guards or safeguarding devices. 4. Employers should ensure all employees have hazard recognition training for all work hazards including emergency procedures/operations to follow. The supervisor disconnected the conveyance line and checked for clogs. After not seeing any apparent clogs, he sent the other employee back into the warehouse to turn on the blower and check the line. The employee was advised by the supervisor to turn off the blower after he received a visual signal from the supervisor. When the employee did not receive the signal from the supervisor, he returned to the work area. Upon arriving to the work area, the employee heard the supervisor yell for him and found him lying on the ground. The supervisor had stepped through a rotten area of the roof and had fallen 19 feet 6 inches to the hard, dirt surface below. Emergency medical services were called to the scene. The supervisor was transported to a local hospital and then transferred to an out-of-state trauma center where he remained until his death on August 24, 2014. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1 Employer worksites should be regularly assessed for structural integrity. Recommendation No. 2: Employers should evaluate and make changes to troubleshooting to prevent exposing employees to hazards. Recommendation No. 3: Employers should require that railing systems or fall protection always be used when working on roofs. Recommendation No. 4: Employers should train new employees on fall protection and proper use of fall protection and require annual refresher training. The crew had already replaced 2 of the 3 antenna arrays and was in the process of setting the third in its place, when they heard a loud pop. The antenna array was 15 feet above the victim on a McKissick "Snatch Block" pulley, Model 419. The supporting shackle suffered a catastrophic failure causing the cable to snap. The antenna array crashed down striking three points on the tower, then the cables amputated the victim's head, and right arm. The employee on the opposite side immediately radioed the ground crew and reported the victim's demise. The ground crew called 911, at 2:22 pm, to report the fatality and ask for assistance in lowering the victim to the ground. They also requested power shut-off for an energized electric line contacting the broken cables. Emergency crews arrived on the scene, minutes later, to begin the tedious process of lowering the victim to the ground. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Employers should ensure that a competent rigger is on site to perform a site hazard assessment before work is performed. Recommendation No. 2: Employers should ensure that all tower technicians are competent and trained on proper equipment use, procedures, and how to safely do their jobs. Recommendation No. 3: Employers should ensure that regular inspection of required antenna replacement equipment is performed before initiating work. Recommendation No. 4: Cell tower employers should ensure that antenna replacement equipment is used properly and according to manufacturers' recommendations. Firefighters 1 and 2 were in the bucket of the tower truck, a third firefighter was on the turntable of the truck opening the valves to access the water, while firefighter 4 was by the back of the truck watching the university film crew tape the introduction. Fundraising participants stood in front of the tower truck and firefighters opened the hose and sprayed them as they sang their school's song. Once the song finished the crowd thanked the firemen and dispersed. As the firefighters disassembled, one witness noticed that while attempting to dock, the bucket moved upward towards power lines instead of lowering. Firefighter 1, who was in control of the bucket, struck the powered electrical line sending a charge throughout the tower truck. Firefighter 1 then slumped down and witnesses heard him moaning. The victim then stood and struck the power line again with the left side of his head. Firefighter 2 who was also in the bucket with Firefighter 1 yelled for help. At 11:44 am, emergency medical services were dispatched to the scene and arrived at 11:48 am. Paramedics treated firefighters 3 and 4 for non-life threatening injuries, and transported them to a local hospital. Firefighter 2 was transported first to a regional hospital then flown to a trauma center with electrical burns. Firefighter 1 was directly airlifted to a trauma center and succumbed to his injuries one month later from electrical burn complications. To prevent future occurrences of similar incidents, the following recommendations have been made: Recommendation No. 1: Whenever possible, avoid operating tower trucks where overhead power lines are present. Recommendation No. 2: Employers should ensure that firefighters wear protective head gear, footwear, and gloves, especially in a bucket where power lines are present. Recommendation No. 3: Employers should designate a spotter on the ground to observe each time the bucket is raised and lowered. Recommendation No.4: Employers should ensure bucket equipment is inspected as recommended by the manufacturer. Recommendation No 5: A communication intercom system from base to bucket should be established when the aerial truck is in use. The movement in the boat caused it to capsize, throwing the victim into the water. The victim was recovered from the bottom of the lake six hours later by the sheriff's dive team. The victim was working alone in the boat, was not wearing a personal floatation device (PFD) and did not have proper training in hazard recognition or water safety. The California Fatality Assessment Control Evaluation (CA/FACE) program investigator concluded that, to help prevent similar incidents from occurring in the future, employers should: 1. Provide personal flotation devices (PFDs) and ensure that workers wear them when working on water. 2. Develop and implement a comprehensive, written Injury and Illness Prevention Program (IIPP) that includes training in hazard recognition and the avoidance of unsafe conditions when working on water and using small watercraft. 3. Have two people on the boat when doing work on water. 4. Have life rings with ropes adequate in length to rescue workers who fall into the water or overboard. 5. Provide proper equipment for operating small, non-motorized watercraft. 6. Consider implementing an integrated pest management (IPM) program for controlling weed growth in lakes that includes a combination of mechanical, operational, biological and chemical (herbicide) techniques, thus reducing the need for working from boats. José was working alone in the boat. Life rings were not available in the boat or onshore with co-workers. José was using a long pole with a net (skimmer) as an oar. The lake was thick with weeds and algae. What Should Be done? Employers should make sure workers are trained in water safety and provide them with PFDs when working over or near water. Two people in a boat can make it more stable. Life rings should be provided and readily available for emergency rescue operations. Using proper equipment (strong and adequately-sized paddles or oars) will help control and navigate small watercraft safely. Employers should consider implementing an integrated pest management (IPM) program to control weed growth.
