Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

They were selectively cutting dead or dying trees which were approximately 20 inches in diameter. While his co-worker was away, the victim cut a tree which broke a snag approximately 30-35 feet up. The snag fell directly on the victim, striking him in the forehead. He was not wearing any type of head protection at the time of the incident. The victim was transported to a nearby hospital and later transferred to a trauma center, but died the same day. The KY FACE investigator concluded that, to prevent similar occurrences, the following precautions should be taken: 1. Ensure that tree fellers properly evaluate the area around timber to be felled so that potential hazards can be identified and appropriate control measures implemented 2. Provide and enforce the use of personal protective equipment (PPE) 3. Ensure that emergency messages can be transmitted quickly 4. Designate a qualified person to conduct regular safety inspections 5. Loggers should attend the Master Logger Program for education regarding logging standards and safety practices. At the time of the accident the 47-year-old male victim was standing on a steel rack resting on a wooden pallet on a forklift. He was not wearing a safety belt or lanyard. As the victim was retrieving tires from overhead bins and placing them on the pallet, the unsecured metal rack and pallet shifted, and he fell approximately 15 feet to the concrete floor below. Emergency medical service arrived within 8 minutes, but the victim was removed from life support 7 days after the accident and pronounced dead as a result of a skull fracture. It was recommended that employers ensure that workers follow established safe working procedures, that all workers be provided with a firmly secured work surface, that all workers be encouraged to participate in workplace safety, and that scheduled and unscheduled worksite safety inspections be conducted routinely. When the sling was removed, the block tipped onto the wooden scaffold. Two co-workers jumped to adjacent scaffolds and were not injured. The victim fell through the hole made by the falling stone in the planks of the scaffold. He suffered severe head injuries in the fall and was assisted in breathing by a trained emergency medical technician on the site until the ambulance arrived. He was then transported to the local city emergency room where he died several hours later. The MA FACE Field Investigator concluded that to prevent similar future occurrences, employers should: 1. Ensure that slings are removed by hand, not with the use of the crane. 2. Consider the use of fall protection systems wherever there exists a possibility of overloading the scaffold system. 3. Develop, implement, and enforce a comprehensive safety program that includes, but is not limited to, a thorough hazard analysis and utilization of controls particular to the job. The deceased was using a John Deere model 2440 tractor equipped with a front bucket and rear grading blade. He had filled the bucket with dirt and was backing away from the dirt pile when the tractor rolled over. The terrain on which the tractor was located was sloping at an angle of 13 to 17 degrees. He had the bucket approximately six feet above ground level as he backed up and turned the vehicle. Skid marks in the dirt indicated that he was backing at an excessive speed for the terrain. Because the bucket was in an elevated position, the tractor's center of gravity was high. These two factors contributed to the tractor turning over. The tractor was not equipped with a roll over protection system (ROPS) and the driver was crushed by the tractor seat and controls. The Colorado Department of Public Health and Environment (CDPHE) investigator concluded that to prevent future similar occurrences, employers should: 1. Equip all tractors with a roll over protection system and a seat belt. 2. Ensure that tractors equipped with buckets are operated with the bucket in the lowest possible position. 3. Develop, implement, and enforce a comprehensive written safety program that includes, but is not limited to, training on all equipment to be utilized. 4. Conduct a job-site survey on a regular basis to identify potential hazards, implement appropriate control measures, and provide subsequent training to employees that specifically addresses all identified hazards. Although the event was unwitnessed, apparently she got off the tractor and was standing in front of a rear tire when the tractor began to roll down the slight incline of the lane, and pinned the victim under the tire. About one-half hour after the victim left the field, the victim's husband (the farmer) was driving a tractor from the field to the farmyard when he saw the victim's tractor standing in the farm lane. He then saw the victim lying on the ground underneath the tire. He backed the tractor off her body, removed blood and field debris from her mouth and attempted CPR. She did not respond, so he placed her in the bucket, drove to the farmyard to call 911, and resumed his CPR efforts. EMS responders arrived, the coroner was called and the victim was pronounced dead at the scene. The FACE investigator concluded that, to prevent similar occurrences, farmers and farm workers should: 1. Shift the tractor into park, set the brakes and turn off the engine before dismounting from the tractor. For tractors without a park position, farmers should shift into neutral, set the brakes and turn off the engine before dismounting. 2. Seek and use the services of organizations and agencies that provide technical assistance and/or adaptive equipment to agricultural workers with disabling conditions. He was lying parallel to the truck, on the ground with his head towards the front of the truck. No chocks were put under the wheels, only the parking brake was used. As the victim removed the last bolt of the U-joint, the drive shaft spun out of his hands and hit the victim in the neck and face area. The truck started rolling down the slope and stopped with the front axle crushing the victim's chest. He was not found until 30-40 minutes later when a relative saw him under the truck and noticed that he was not moving. A tractor front end loader was used to raise the front of the truck to access the victim. CPR was attempted, but he was dead at the scene. It was determined later that the parking brake on this truck was connected to the transmission, and acted through the drive shaft. This break became ineffective when the driveshaft was disconnected from the transmission and the truck was free to roll backwards down the incline. The victim obviously assumed that the parking brake locked the rear wheels. Recommendations following our investigation were as follows: 1. Workers should always chock the wheels when working on a vehicle that is left on an incline. 2. Trucks should be repaired in a shop equipped to repair large vehicles. He was pronounced brain dead 43 hours later due to blunt force head trauma. The victim was an oil gauger for a company that transports crude oil. His job was to obtain samples of the oil to check its quality. On the day of the incident he was to check the oil at a site located about 100 miles from his office. He was an experienced gauger and had been to this site many times before. This company owned four cylinder-shaped oil tanks. Three of the tanks were 15 feet high and one was 12 feet high with a diameter of 8 feet AB - each tank had a ladder welded to the side. There were no guardrails or other safety devices on the tops of the tanks, and the victim was not using any fall protection equipment. As was his usual practice, the victim was working alone and had ascended the ladder of the 12-foot tank to take a sample of oil through a 12-inch opening on the top of the tank. The owner of the company, who had been indoors while the victim was working, went outside and found him lying on the ground beside the tank. Although there were no witnesses to the fatal incident, evidence of fresh oil on the bottom of one of his boots and an oil smear on the top of the tank suggest that he slipped on the oily residue, lost his balance and fell. Upon finding the victim the owner called 911. Rescue personnel arrived within five minutes. The victim was pronounced dead at the scene. Toxicology results later revealed that the victim had a blood alcohol concentration of 0.17. In order to prevent similar incidents from occurring, FACE investigators recommend: 1. Employers should follow the OSHA guidelines for personal protective equipment 2. Sites should be equipped with safety devices such as nonskid surfaces and railings on top of the tanks, and the surfaces on top should be kept free of oily residue 3. Employers should develop, implement, and enforce a written safety program which includes worker training in recognizing, avoiding and abating hazards in the workplace 4. Workers should refrain from drinking alcoholic beverages while on the job, as it may hinder judgment, affect balance and thus contribute to injury. In order to gain additional space in the truck, the victim turned on the switch to advance the platen into the compactor body, beyond the normal automatic stroke. He then entered the hopper, apparently to remove some debris, but accidentally activated the platen return switch, causing the platen to retract suddenly, pinning him. The coworker immediately threw the shutdown switch and called for help. The victim was pronounced dead at the scene due to crushing injuries. It is recommended that employers ensure that their workers are following appropriate hazardous energy control procedures including lockout and tagout operations, that workers are familiar with all the features of the equipment they are using, that manufacturers of automatically operated equipment consider installing redundant safety features, and that all equipment be checked on delivery so that it is certain to be operating correctly. The rectangular pond covered about 2 acres in size. The crew, on which the victim was working, was cutting brush with a chain saw, and weeds and grass with a tractor pulling a brush hog. The brush hog was equipped with two heavy duty cutting blades which produced a 6-foot-wide cut. The victim was removing brush on the bank of the pond and had a rope tied around his waist which was being held by his coworker. The victim called to his helper to assist him with the chain saw. Leaving the rope on the ground, the helper went to the victim to assist him. At about that time the spotter for the tractor came upon several gas cans in the path that the tractor was cutting, and he began removing these cans to a refuse truck. The tractor driver continued cutting and ran over the rope lying in the grass. The victim was jerked up the bank and into the air toward the tractor, striking the metal cab enclosure of the tractor headfirst. He was pronounced dead at the scene due to multiple traumas. It is recommended that a reevaluation be made of employees working in the proximity to operating equipment, that operators stop equipment when their spotters are out of sight, that full body harnesses be worn when tied off, and that unscheduled safety checks be made at all job sites. Fatality Assessment and Control Evaluation (FACE) Program AB - A 39-year-old woman employed as a punch press operator at a company manufacturing chassis and cabinet components for personal computers died as a result of being struck and pinned by a large metal scrap bin. The company employed about 725 workers at this site and had a written safety program. The victim operated a four-station punch press, located in line with other presses. At the time of the accident, the victim was working at her station when a forklift rounded the corner of the aisle, traveling in reverse at a high rate of speed. The forklift struck a metal scrap bin, throwing it toward her work area. The bin hit the press, rebounded toward the forklift, was hit again, and shoved against the corner of the victim's press, crushing the victim against the press. The victim died as a result of multiple crush injuries. Recommendations are made to ensure that work areas in manufacturing facilities are guarded from vehicle travel ways by installing physical barriers, and that forklifts are operated at safe speeds and in the forward direction when visibility permits. At the time of the accident, three tree trimming crews had just completed cleanup of an area where they had worked for about 1.5 hours to clear a tree about to fall across powerlines. One of the truck drivers instructed a second groundman to secure the push pole in the travel position underneath the truck bed. On trying to do so, the pole hit the hydraulic lines at the oil reserve unit of the articulating boom. The victim attempted to assist by climbing under the truck in front of the dual right rear wheels. The victim called to the groundman to go ahead, but the truck driver thought the message was for him and pulled the truck forward, running over the victim. The victim died of crushing head and chest injuries. Recommendations include being certain that workers follow established safe work procedures, that workers be encouraged to actively participate in workplace safety, and that an established line of supervision is maintained at job sites. The machine was a single person lift on wheels with a hydraulic pump reservoir operated by a 12 volt battery. The scaffold did not have any type of a brake system, but outriggers were available to stabilize it. At the time of the incident, the outriggers were not being used to increase the stability of the lift. The victim may have pushed away from the hoist system causing the lift to tip back on its wheels and fall over. The victim was thrown to the floor and the lift fell on top of an electric cart which was parked on the floor. He was held at the hospital where he died of complications twenty days following the incident. The MA FACE Program concluded that to prevent similar future occurrences, employers should: 1. Implement a personal protective equipment inspection program that ensures the removal of defective equipment from service. 2. Ensure that all employees who are subjected to fall hazards in the course of their employment are suitably trained in fall hazard recognition and the use of personal protective equipment. 3. Ensure that surfaces from which employees might work be structurally adequate and suitable for the task. He got off the tractor at home and used a cordless phone to call for emergency services. Emergency medical technicians (EMT's) found him sitting in a chair at home. The victim spoke a few words to the EMT's, telling them he had been run over by the tractor, and specifying the hospital to take him to. He died in the ambulance, while en route to the hospital. The FACE investigator concluded that, to prevent similar occurrences, farm tractor operators should: 1. Place the transmission in park. 2. Evaluate all hazards in area. 3. The victim was working alone combining beans in a remote field. He had purchased a used early 1970's self-propelled combine the previous year. This was the first time he was using it in the field. He had difficulty operating the combine AB - it did not separate the beans properly and the cutting height was too high at times and too low in some places where he had been combining. The combine header on the left side had become clogged with field dirt while traveling too close to the ground. The farmer had apparently disengaged the header using a control lever in the cab, then climbed down and proceeded to clean the sickle and auger of dirt. He left the engine running because it had a bad battery. Apparently the header slipped into gear on its own while he was cleaning the dirt. The reel and auger had thrown him around to the right side and pulled his left leg into the auger. The chain driving the reel and auger broke while he was caught between the reel and auger leaving the sickle still running. He was found unconscious lying on the sickle with significant leg injury and loss of blood. An emergency crew used the jaws of life to cut through the reel to access the victim, but he died enroute to the hospital. Recommendations following our investigation were as follows: 1. The operator should disengage the header and other moving parts as well as shut off the engine before cleaning, maintenance, or working close to the combine. 2. Machinery dealers should ensure that their machines are in safe working order when sold and that the operator is familiar with safety and operation of the machine. 3. Owners/operators should keep combines in good working order so they can shut off the engine during cleaning or routine maintenance of the machine. TY - Journal Article AU - Gillen, Matthew AU - Harrison, Robert AB - The surveillance and investigation of work related homicides conducted through the California Fatality Assessment and Control Evaluation (FACE) program were discussed. Surveillance was based on multiple sources, such as coroners' reports, death certificates, newspaper articles, OSHA reports, and employer reports. Investigations of selected cases were also conducted. Surveillance and investigation efforts were limited to Los Angeles and surrounding counties. From 1992 to 1994, the California FACE program discovered 444 work related fatalities and performed 31 on site investigations. Over this period, 47% of the occupational fatalities were attributed to homicide. Despite the fact that work related homicide was the leading cause of occupational fatality among women, 92% of workplace homicide victims were men. Most homicide victims were in the 25 to 54 year age range. Compared to national data, more of the Los Angeles homicide victims were Asian/Pacific Islanders or African Americans. Of the workplace homicides reported, 46% involved robberies and 85% involved firearms. Almost two thirds of the work related homicides occurred in retail and service establishments. Workers in technical, sales, administrative, and service fields were most often affected. The overall rate of workplace homicides in Los Angeles was 1.87 pre 100,000 individuals. Elevated homicide rates, relative to national values, were reported for taxicab establishments, liquor stores, nonstore retail, and protective services. Pilot testing was being conducted for the California FACE program's protocol for workplace homicide investigation. The authors conclude that continued surveillance and workplace investigation are needed to identify high risk occupations and design effective interventions for the reduction of work related homicides. Homicides resulting from robberies are the top priority. The tractor had a wheat drill (planter) attached to the tow bar. The wheat drill was pulled over the victim's lower extremities, abdominal region and thoracic region. The farm owner noticed the driverless tractor turning in circles in the field. When he investigated, he discovered the victim. A local ambulance was called and the victim was transported to the local hospital where he expired in the emergency room. The Colorado Department of Public Health and Environment (CDPHE) investigator concluded that to prevent future similar occurrences, employers should: 1. Ensure that employees are familiar with all safety features on equipment that they will operate. 2. Post bilingual signs on equipment to designate that a safety hazard exists. 3. Survey the work site to identify hazards. All employees should then be informed of the possible hazards and encouraged to report any unsafe work conditions. The tractor was turned off, but apparently left in third gear. After completing some repairs, he stood next to the tractor in front of the right rear tire, and turned the key to start the tractor so he could raise the bucket. The tractor lurched forward and ran over the victim. The victim's son had been working nearby in the barn and came to investigate when he heard the tractor engine running at a high speed. He found his father lying injured on the ground. Before he died, the victim briefly told his son how the incident had occurred. Emergency medical services were summoned and arrived within fifteen minutes. The victim was pronounced dead at the scene. The FACE investigator concluded that, to prevent similar occurrences, farm tractor operators should: 1. Be seated in the operator's seat before starting a tractor. 2. Place the transmission in neutral or park and follow the manufacturer's starting procedure. 3. Keep the tractor's starting system in repair. In addition, farmers and farm workers with functional limitations caused by illness or injury should: 4. Seek and use the services of organizations and agencies that provide technical assistance and/or adaptive equipment to agricultural workers with disabling conditions.
