Description:This report concerns the death of a 47-year-old man who was employed by an interstate powerline tree trimming company with 250 workers. The local electrical cooperative had contracted the company to clear powerline right of ways. At the time of the accident, three tree trimming crews had just completed cleanup of an area where they had worked for about 1.5 hours to clear a tree about to fall across powerlines. One of the truck drivers instructed a second groundman to secure the push pole in the travel position underneath the truck bed. On trying to do so, the pole hit the hydraulic lines at the oil reserve unit of the articulating boom. The victim attempted to assist by climbing under the truck in front of the dual right rear wheels. The victim called to the groundman to go ahead, but the truck driver thought the message was for him and pulled the truck forward, running over the victim. The victim died of crushing head and chest injuries. Recommendations include being certain that workers follow established safe work procedures, that workers be encouraged to actively participate in workplace safety, and that an established line of supervision is maintained at job sites.
