Description:The case of an assistant warehouse manager who fell to his death from a forklift suspended pallet was examined. The victim worked for a tire warehouse which had written safety procedures that were presented to new employees during orientation training. On the job training was provided, forklift operators attended safety training, and monthly safety/quality inspection reports were completed by managers and assistant managers. At the time of the accident the 47-year-old male victim was standing on a steel rack resting on a wooden pallet on a forklift. He was not wearing a safety belt or lanyard. As the victim was retrieving tires from overhead bins and placing them on the pallet, the unsecured metal rack and pallet shifted, and he fell approximately 15 feet to the concrete floor below. Emergency medical service arrived within 8 minutes, but the victim was removed from life support 7 days after the accident and pronounced dead as a result of a skull fracture. It was recommended that employers ensure that workers follow established safe working procedures, that all workers be provided with a firmly secured work surface, that all workers be encouraged to participate in workplace safety, and that scheduled and unscheduled worksite safety inspections be conducted routinely.
