"This supplement carries final figures on the reported incidence of notifiable diseases in 1966 based on summaries submitted to the National Communicable Disease Center by the individual States through the National Morbidity Reporting System and on data collected by the Tuberculosis Program, the Venereal Disease Program and two surveillance units of the Epidemiology Program - the Neurotropic Viral Disease Unit and the Rabies Control Unit. The illustrative materials depicting disease trends are for the most part based on charts and graphs published in the 1966 issues of the Morbidity and Mortality Weekly Report. Data For these charts are from the National Morbidity Reporting Sys tem, National Surveillance Programs and the National Vital Statistics Division. The population estimates used in computation of rates are from the Bureau of the Census, Current Population Reports, Series P -25." – p. 3
