This cardiovascular disease-specific guide serves as a companion resource to Surveillance and Evaluation Data Resources for Comprehensive Tobacco Control Programs, which focused on cigarette smoking and smoking cessation, produced by the CDC’s Office of Smoking and Health in June 2014.   Surveillance and Evaluation Data Resources for Heart Disease and Stroke Prevention Programs is an at-a-glance compilation of data sources useful for heart disease and stroke prevention programs conducting policy or data surveillance and/or evaluation. This guide addresses the broad spectrum of programs supported by the Division for Heart Disease and Stroke Prevention and not all sources are applicable to all programs. It is meant to be used by program managers and evaluators in the planning and evaluation stages of heart disease and stroke prevention programs. When and how to use the available resources is addressed in other evaluation resources such as Using Indicators for Program Planning and Evaluation, and during technical assistance events.
