Description:The Partnerships to Improve Community Health (PICH) and Racial and Ethnic Approaches to Community Health (REACH) (2014) programs are a 3–year initiative aimed at improving the health of communities and reducing the prevalence of chronic disease. PICH and REACH builds on a body of knowledge developed through previously funded Centers for Disease Control and Prevention initiatives and encourages collaborations with a multi-sectoral coalition to implement sustainable changes in communities where people live, learn, work, and play. This document provides an overview of focus areas, top priority interventions and current awardee projects.
