• Description:
  Purpose: Facilities and organizations can use this table for their own internal records to document measles immunity for staff, volunteers, clients or residents, program participants, students, campers, children, or other. If there is a person with measles symptoms at the facility/organization, administrators can give this information to the health department to assist in their response. The health department can use the information to determine whether anyone without measles immunity should be temporarily excluded from the facility/organization to protect their health and prevent further spread of measles.
