      Ill traveler card – required evaluation (for travelers with Ebola symptoms and risk exposures) For use with travelers who might have been exposed to patients with Ebola and are sick with symptoms consistent with Ebola infection and require further evaluation.

      You have been given this card because you might have been exposed to patients with Ebola, and because you are sick with symptoms consistent with Ebola infection. This does not mean that you have Ebola, but further evaluation is needed.

      A note to the doctor: The patient giving you this card has potentially been exposed to patients with Ebola and might have symptoms consistent with Ebola.
