Detection of adriamycin on surfaces and skin.

  Information was sought about dermal exposure to health care workers to adriamycin (23214928), an antineoplastic agent. Two hospitals and a privately owned medical clinic granted permission to monitor personnel and work surfaces. Adriamycin was selected not only because of its wide use but also due to physical properties which allowed detection. Forty two observations were made of personnel administering adriamycin. Only one time was fluorescence noticed on the hands and only on two occasions was fluorescence noted on the clothing of the administering personnel. A total of 45 observations of work surfaces were taken. Of the 26 of these taken on surfaces in pharmacy areas, contamination was noted in four. Of ten observations of surfaces make in administering rooms, only one contaminated spot was found. In these hospitals there were strict procedures for the safe mixing, transferring, administering, and cleanup of spills, as well as certain qualifications which individuals had to meet before they were allowed the responsibility of dealing with these drugs. On the other hand, even though the health care personnel were aware of the hazards, no written procedures existed for the safe handling of the drug at the privately owned medical clinic. The author recommends that such procedures be written down, that all mixing and preparation be carried out in a chemical fume hood, that protective clothing and gloves be worn, that observations be carried out periodically to check for compliance, and that all spills be reported and made a part of the worker's permanent record.
