Description:The findings itemized in this report should be examined hand-in-hand with operator self-report data (from the associated survey or additional follow-up as needed). It is possible that some of the situations the authors identified as problematic may not be of concern to operators or represent adjustments made by operators or managers that have some importance unknown to the investigators. On the other hand, operators' response may add priority to some of the concerns or point out related problems that failed to notice. Similarly, in the installation of proposed solutions, it would be wise to proceed in a systematic step-wise fashion, evaluating their effectiveness by operator trial before large scale changes are made.
