Worksite health promotion utilization health risk appraisal -- the painters project.

  A project was described for assessing the utility of the Health Risk Appraisal (HRA) for industrial painters, a group of blue collar workers whose workplace exposures were known to interact with lifestyle behaviors in increasing the risks of illness and injury. Responses to a questionnaire designed to identify health problems related to work were collected in surveys in Fort Worth, Texas (131 persons), and Memphis, Tennessee (179 persons). In addition, occupational histories were obtained and medical testing was performed. General comments on the HRA instrument included concerns of workers about morbidity (fear of lost workdays and income) in distinction to the focus of the HRA on mortality, attention to workplace health hazards, the need to affect the behavior of the group rather than of individuals, and meaningful and appropriate HRA output (more narrative output and less focus on risk estimates or wellness scores). Specific concerns about technical aspects of the system were addressed with respect to proposed revisions of some of the questions. Problems with the computer software and data entry were documented. The authors recommend that future work should address concerns regarding workplace morbidity, utilize a job exposure linkage system for identifying hazardous chemicals, revise the HRA instrument to make the questions more appropriate for blue collar workers, provide improved software, and develop guidelines to insure that the HRA instrument is adequately used in an appropriate context.
