Utilization of a systems approach in analyzing epidemiologic case studies - control of hazardous energy.

  The systems analysis of options available to those responsible for designing safe work procedures involving the control of hazardous energy sources in the workplace was discussed; the focus was on applications to electrocutions evaluated by NIOSH as part of the Fatal Accident Circumstances and Epidemiology program. The logic tree analysis used by NIOSH outlined the decision criteria for providing worker protection from hazardous energy during maintenance and servicing tasks; factors included the magnitude of energy, the proximity of the energy source to the work area, and the existence of proven methods to perform the task safely with hazardous energy present. Six fatal occupational accidents were discussed; the cases demonstrated that intervention strategies developed to minimize electrocution risk needed to address all aspects of the logic tree analysis. Each incident involved the violation of options identified by logic tree analysis to contribute to the desired event. The authors conclude that the systems approach to analysis of safety practices aids in the identification of many variable contributing factors that can influence overall system performance.
