A retrospective study of clinical, physiologic and immunologic changes in workers exposed to toluene diisocyanate.

• Description:
  Clinical, physiologic and immunologic changes are studied in 30 of 300 workers exposed to toluene-diisocyanate (584849) in a plant operating continuously since 1956. The average concentration of the chemical in the workroom air initially was 0.06 ppm and was reduced to the present value of 0.004 ppm. The onset of sensitization of the 24 persons experiencing clinical reactions ranged from 2-14 years. Six others who were immediately hypersensitive to the lowest exposure showed no symptoms after being removed from contact. One type of response was seen in those who became immunologically "anergic" and no longer reacted to the chemical's antigens; these persons were also symptom free. Some workers showed protective (IgG) antibodies but still experienced bronchoconstriction. They responded promptly to bronchodilators and are considered to be histamine responders. The importance of medical management of industrial workers is emphasized. [Description provided by NIOSH]
