  Prevention and early diagnosis are population based methods designed to manage health care before illness advances to the point where its manifestations demand the patient's attention and lead him to seek care. Prevention requires thinking on a large scale. Primary prevention eliminates the cause of the disease and therefore the disease. Secondary prevention eliminates the effects of advanced disease through early diagnosis and effective therapy. In the occupational setting, environmental surveillance is a method for achieving primary prevention, and medical surveillance is a method for both primary and secondary prevention. Some of the principles of surveillance are discussed and some of the difficulties in applying the principles are pointed out.
