Toolbox Talk Guide: Mechanic Killed by Excavator Bucket

Our safety talk today is about a 47-year-old mechanic who was killed when an excavator bucket crushed him. The conveyor on a rock crusher had stopped, so the mechanic stood on the conveyor to attach a chain to lift it for repair. The excavator operator leaned forward to see where the mechanic was. When he sat back down, his raincoat caught on the control handle for the boom. This caused the boom to swing and drop down onto the mechanic, crushing him to death. So here are some ways we can prevent something like this from happening where we work. Avoid wearing loose clothing when operating machinery. Make sure that you do not stand in pinch points. Use a signal person for limited sight lifts or picks. Management or I need to make sure everyone is properly trained to identify hazards and best practices during lifting operations. Management or I should inspect sites, assure hazards are assessed and safe procedures are followed. ASK: "Does anyone have more ideas or comments to share?" Pause for discussion. Then see if there are ways to take action. END WITH ACTION PLAN (ideas for what to ask or say). "Is there a task today that has not been assessed for hazards?" "What would you do if you saw someone position themselves in a pinch point?" Discuss similar or other potential machinery hazards at your current site. Express your commitment to training workers on hazard assessment and best practices during lifting operations. Commit to follow-up at the next safety talk. Description provided by NIOSH
