Description:Standardized criteria for the pathological diagnosis of asbestosis and a system for grading the severity and extent of this disease were developed. Material used for testing the grading system was screened from several hundred cases by two pathologists. Two sections were assessed in each of 20 cases. Slide 1 of each pair was read twice by each of nine pathologists, whereas slide 2 was read only once by each. Each slide was read independently by every pathologist; about 5 minutes was allotted for each slide. Analysis was undertaken separately for severity and extent of disease as a method for evaluating variations among pathologists. Distribution of severity gradings did not differ markedly. Three statistics were chosen to measure reproducibility: percentage of exact agreement, percentage of agreement to within a degree of significance, and degree of difference. Intra pathologist reproducibility on severity was much better than inter pathologist reproducibility. The mean percentage of exact agreement was 71, compared with 57 percent for the inter pathologist evaluation. Pathologists rarely differed by more than one category in repeated readings. The results for extent almost duplicated those for severity. The authors conclude that the proposed grading scheme has acceptable inter observer and intra observer variability.
