Description:Perfusion lung scans were used to detect avascular areas in the lungs of coal miners. Thirty seven coal miners, 14 with complicated pneumoconiosis and 21 with simple pneumoconiosis, were intravenously injected with a suspension of aggregated albumin-131-iodide. Three photoscans, one each in the anteroposterior, right, and left lateral positions, were obtained and examined for abnormalities by two observers. The scans were compared to standard chest radiographs of each subject. In all 14 subjects with complicated pneumoconiosis, conglomerate masses in the chest films were also detected as avascular zones in the scans. Bullae and compensatory emphysema, common in this group, were easier to detect in the scans than in the radiographs. In only two subjects with simple pneumoconiosis was there a scan abnormality explicable on the basis of some other condition: one subject had very early conglomeration; the other had a heavy nodular infiltrate. The authors conclude that the vascular bed of the lung is normal in most patients with simple pneumoconiosis and becomes abnormal in the presence of conglomeration and bullous disease. The interpretation of avascular areas on the scan is greatly assisted by a combined clinical, physiologic, and radiographic approach. [Description provided by NIOSH]
