Description:A case report of occupational asthma due to toluene diisocyanate (26471625) (TDI) is described. A 43 year old nonsmoking female complained of throat irritation and a nonproductive cough after working in a polyester resin manufacturing factory for 2.5 years. Detailed immunologic studies including those for p-tolyl isocyanate IgE antibodies and the presence of a leukocyte inhibition factor for isocyanate antigen were negative. Methacholine challenges also were negative. A dose related response to TDI was observed on bronchial provocation testing using TDI concentrations of 0.0165 and 0.030 parts per million. The authors conclude that airway response to TDI may not always be accompanied by hyperactivity to methacholine, and that screening procedures utilizing methacholine challenges may not always identify sensitized workers.
