Description:Due to concern about possible nosocomial/occupational transmission of Mycobacterium-tuberculosis in hospitals in developing countries, a joint activity was begun to develop methods to control occupational exposure among health care workers (HCWs) at the Chiang Rai Hospital, Thailand. A survey was conducted to assess the facilities, and recommendations for preventing transmission of M- tuberculosis were developed. Continuous management was discussed as a method to assess the effectiveness of the infection control program in an area with a high incidence of tuberculosis. The recommended measures for M-tuberculosis infection control focused on three areas: hospital employees; patients; and environmental or engineering interventions. For hospital employees, recommended interventions included worker training and education, tuberculin skin testing, prevention of active tuberculosis, and personal respiratory protection. Recommended interventions for patients included identification of infectious tuberculosis patients, management of suspected tuberculosis patients, diagnostic evaluation, management of confirmed cases, and patient training and education. Specific engineering control measures were recommended for each area of the hospital, including general ventilation, exhaust ventilation, and air cleaning.
