Description:A program for managing the respiratory health of aluminum workers was summarized. The program was developed by Aluminum Company of America (Alcoa), Pittsburgh, Pennsylvania after studies had shown that workers exposed to high concentrations of bauxite (1318167) or alumina (1344281) dust for 20 of more years had significant decrements in 1 second forced expiratory volume (FEV1). The The program was designed to provide adequate respiratory protection against dust overexposure and to establish a pulmonary surveillance system to evaluate the effectiveness of the respiratory protection program. The pulmonary surveillance program involved periodic spirometric testing, creation of computer software to provide automated storage and access to the spirometric data, development of appropriate criteria incorporated in the software for screening of the spirometric test results, and periodic analyses of the data to evaluate the effectiveness of the surveillance program. The criterion for determining whether a worker had suffered a pulmonary impairment was a decrement in FEV1 or forced vital capacity (FVC) of greater than 250 milliliters (ml) plus an annual decrease of 50ml. Any employee who demonstrated an FEV1 or FVC less than 80% of predicted, but which was greater than 60% of predicted, was referred to the medical department for additional testing.
