  Three fatalities associated with confined spaces, two with electrical hazards, and one occurring in a mining equipment repair shop were reported. Confined space deaths involved a 58 year old steel worker who fell into an industrial waste pit containing light tars, light oils, water, and naphthalene (91203) at 160 to 180 degrees-F; a 22 year old worker cleaning a toluene (108883) storage tank; and a worker who had used an epoxy coating containing 2- nitropropane (79469) and coal tar pitch to cover a valve on an underground water line in an enclosed service vault. A fireman was also killed in an explosion occurring when rescuers cut open the toluene tank. Deaths related to electrical hazards included an 18 year old laborer electrocuted when a metal pole he was carrying contacted a 7200 volt power line, and a 50 year old utility worker electrocuted when he elevated his aerial bucket into a 7200 volt power line. The other case was that of a 50 year old welder who was working in a mining equipment repair facility. He was working with his back to a coal car which had been turned on its side. A cutting torch had been used to remove the wheel assemblies from the cart. When the cart had been brought inside it contained a block of frozen material which, due to increased temperature inside and heat from the torch, melted enough to slide from the cart, pinning the worker against the grinder he was using. The welder died 3 days later from his injuries.
