Osteonecrosis as it concerns the insurance underwriter.

  Precautions against osteonecrosis established by one insurance company for acceptance of insurance risk for compressed air or high pressure workers were described. A preemployment examination was required for employees exposed to increased atmospheric pressure or compressed air. This examination included a chest X-ray, anterior X- rays of both shoulders and hips, anterior and lateral X-rays of both knees and sickle cell trait determination. Annual physical examinations were required, including six joint X-rays and a chest X- ray every 2 years. To further safeguard the employees and protect the insurance carriers, a company engaged in compressed air work had to have a qualified air master under the direct supervision of the physician in charge. This air master had to be present any time that employees were working under compressed air, during compression, and during decompression. The decompression tables as printed in the U.S. Department of Labor's Safety and Health Regulations for Construction were to be followed unless a more stringent set of regulations existed in the state where the work was being carried on. Payments by one insurance carrier for osteonecrosis claims were briefly discussed.
