    Heart disease is the leading cause of death in Georgia and non-Hispanic Black people experience higher prevalence of cardiovascular disease (CVD) than other racial and ethnic groups. The Applied Research and Evaluation Branch collaborated with the Grady Health Camp Creek Comprehensive Center to understand the unique perspectives of patients living with CVD and to examine facilitators and barriers to collecting health equity indicators (HEIs) in an outpatient setting.

    Mixed methods were used to collect health equity data from 60 patients in five SDOH areas: psychosocial pathways, racism, neighborhood characteristics, socioeconomic factors, and healthcare access. Demographic and cardiovascular risk data (diabetes, hypertension, and obesity) were also collected. Descriptive statistics and chi-square tests were performed to better understand the association between social vulnerability and CVD risk. Interviews were conducted with patients to obtain narratives on patients’ lived experiences with CVD. Interviews with clinicians were also conducted to examine facilitators and barriers to collecting HEIs.
