Description:A 52 year old electrical technician, employed by a high voltage power transmission and distribution equipment company, was electrocuted while testing circuits. A metal cabinet containing a transformer, a capacitor, and two resistors had been repaired and sent to the quality assurance department for a final check. Electricity was being supplied to the electrical components in the cabinet from a variable output transformer. The technician determined that the rectifiers in the control circuit were shorted, replaced both rectifiers, and continued to follow the testing procedure check list. At the point where he was to measure the resistance of the two 5000 ohm resistors with an ohmmeter, he apparently reached into the metal cabinet with his left hand and attempted to remove one of the resistors. When he touched the resistor he was electrocuted. Recommendations arising from this accident include: a reevaluation of the testing procedure and check list for this task which would eliminate all unnecessary exposure to energized equipment
following the testing procedure check list and safe job procedure for repairing and testing a cabinet containing power transmission and distribution equipment
redesigning the variable output transformer so that it is controlled by one on/off switch
and designing power transmission and distribution equipment to incorporate safety considerations to protect workers who maintain, repair, and test these cabinets.
