Description:A 23 year old groundman employed by an electrical contracting corporation was electrocuted while helping to plumb and anchor a previously set utility pole. Electric distribution lines and a transformer were to be transferred from the old pole to the new one. Before the pole was moved into a plumb position, a lineman attached a temporary insulator on the side at the top of the new pole. A saddle attached to the primary line was removed, but two squeeze on clamps remained on the primary line, which was placed on top of the insulator and secured with aluminum wire. The clamps were 6 to 8 inches apart on either side of the insulator, preventing sliding of the primary line through the tie wire. A groundman operating a winch to plumb the pole moved the top of the pole about 2 to 3 feet, which increased tension on the primary line. The aluminum tie wire broke and the primary line moved from the insulator onto the top of the new pole, energizing it. The victim was moving a second guy into position, and walked between the new and old poles carrying the second guy. He passed it around the new pole, bringing it into contact with the copper butt ground wire on the side of the pole, and was electrocuted. The groundman operating the winch was also seriously injured. Recommendations include: identification of all possible safety hazards prior to beginning work
and removal or positioning of squeeze on clamps so as not to contact the insulator.
