Description:A fatal accident circumstances and epidemiology (FACE) report of an electrocution in North Carolina was presented. The report was part of the NIOSH FACE project to collect data on electricity or confined space/related accidents involving fatalities. A 24 year old male employee of an electronics component manufacturing company was electrocuted while explaining a test procedure to a fellow employee on September 19, 1985. He reached into an energized component to show how to hook up to the bus bars and inadvertently touched two of the bus bars, coming into contact with 380 Volts. Burns were noted on the fingers of each hand where he contacted the bus bars. He did not follow standard operating procedures
he did not verify that voltage was not present. Employees were instructed to assume that voltage was present and to always check for voltage using a functioning and properly calibrated instrument. The victim had been employed by the firm for 6 months and was in the last week of the third stage of a three phase training program. Recommendations include stressing hazard awareness at safety meetings and evaluating testing programs to eliminate unnecessary exposures to unguarded electrical parts.
