Description:A fatal accident circumstances and epidemiology (FACE) report on an electrocution in Ohio was presented. The report was part of the NIOSH FACE project to collect data on electricity and confined space/related accidents involving fatalities. On August 19, 1985, a power line repairman was electrocuted while attempting to open a pole mounted, ground level air switch on a three phase, 69 kiloVolt power line. Evidence at the accident site indicated that a flash over arc occurred between the middle phase of the transmission line and the air switch assembly. This contact caused a fault current to flow down the common ground wire on the pole. The common wire burned off at the ground rod at the base of the pole. The broken connection interrupted the path to ground for the current. The victim supplied a path to ground when he grasped the handle. The author notes that since the accident, the company has recommended using rubber gloves and overshoes when operating pole mounted switches and is studying the feasibility of placing ground mats at the base of poles with air switches. He recommends inspecting new installations prior to entering them into service and providing training in interpreting symbols on blueprints.
