21-Year-Old Technician Electrocuted in Indiana

Description:A 21 year old male plastic extrusion machine operator was electrocuted after he reenergized the corona treater on his machine. The corona treatment, to allow printing on the plastic sheet, required charging a steel treater bar with a direct current (DC). A corona was established between a grounded conductive roller and the treater bar, through which the plastic sheet was passed. The plastic treater was designed to operate at 9600 hertz (Hz). The treater bar transformer was deenergized while a bonding strap was installed between the machine and an electrical panelboard. The extrusion machine was still producing plastic sheet, but this was not being corona treated. The machine operator reactivated the transformer; while he was standing on a metal step on the operator's side of the machine he touched the treater wire/cable or the treater bar with his right hand, coming into contact with a potential 10 kilovolt DC, 9600Hz source of electricity. The recommendations include: enclosing insulated conductors supplying power to this equipment in accordance with the manufacturer's recommendations; developing and implementing a comprehensive occupational safety program; testing equipment after making adjustments or modifications to it to assure that the equipment is operating safely before placing it back into full service; and adequately training and equipping personnel to install, operate, maintain and repair plastic extrusion and corona treater equipment.
