Two Electrical Contractor Employees Electrocuted in Kentucky

Description:While installing electrical service to a mobile home, two electrical contractor employees were electrocuted. A three man crew consisting of a foreman, a lineman and a groundman had covered all three power lines with insulating hose, set a mid span pole, and attached a cross bar to that pole in preparation for the installation of a transformer. The crew foreman and lineman were working from the bucket of an aerial truck with the groundman assisting them. After attaching the cross bar to the pole, the victims removed the insulating hoses from the power lines. When the victims tried to lower the bucket, the bucket raised, bringing the foreman and lineman into contact with the power lines. Some time passed before the groundman became aware of the problem and lowered the bucket to the ground. Electrocution was ruled the cause of death for both men. After the accident, the upper boom was found to be not operating properly. The boom controller was also considered to be awkward to use. Recommendations arising from this accident included: emphasizing the importance of employees following safe job procedures including the testing and reporting of any defective equipment; and an evaluation of the boom control system so that the possibility of human error could be prevented by more careful engineering design.
