Description:A stagehand, working above the ceiling of the entertainment center, was electrocuted when he contacted an exposed electrical wire protruding from an uncovered junction box on one side of the catwalk. Previously this wire had been used to supply electricity to a nearby mercury vapor light. When this light was disconnected the cover plate of the junction box was not replaced. The victim was an independent contractor and member of the local stagehands' union. While safety was discussed at local union meetings, there was no formal safety program. All training was done on the job. Cardiopulmonary resuscitation was begun in less than 1 minute following the accident, however, the man had to be removed from the catwalk before defibrillation and other advanced cardiac life support measures could be applied. The victim was pronounced dead due to electrocution. Recommendations arising from this accident include the inspection on a periodic basis of the electrical system above the ceiling of the facility, and the specific training of stagehands in electrical safety.
