Electric Company Lineman Electrocuted in Indiana: FACE 86-03

  A 35 year old serviceman was electrocuted while installing a guy wire to a pole. A conductor was being installed between two adjacent poles by a four man crew. The span between the poles was 341 feet with a potential voltage of 7200 volts to ground. The victim and the foreman were each in truck mounted aerial buckets on their respective poles. The victim's pole supported a three phase power system. All conductors between the two poles were deenergized, and the bottom phase of the three phase system was covered with an insulated rubber line hose. After connecting the conductor to the pole, the victim energized the lines, lowered the aerial bucket to the ground and attached a guy wire to the outside of the aerial bucket. He then raised himself into position to attach the guy wire to the pole. He was wearing no insulated gloves or sleeves although both were present in the bucket. His back apparently contacted a live conductor while he was grasping the guy wire in his hands, and he was electrocuted. Recommendations arising from this accident include: positioning of trucks for aerial buckets so that linemen are not required to work through power lines to perform the work task ; wearing required personal protective equipment ; recognition of all possible phase to phase or phase to ground contacts by employees ; covering all exposed live conductors with rubber hoses, boots or blankets ; and evaluating the procedure for installing guy wires to poles.
