Description:A 34 year old superintendent of manufacturing at a facility which produced mulch fibers from wood chips was electrocuted while attempting to find an electrical fault. On a Sunday, the superintendent was called at home and told that the 800 horsepower motor of a refiner would not start. He and two other workers began to search for the electrical failure in a control panel located on the exterior of the building. The superintendent then checked a control panel box on the inside of the facility, by opening the double metal doors and visually checking the relays while standing on a metal stool. The superintendent requested and received a flashlight. He then received an electric shock and asked for help. On his second attempt, the foreman succeeded in pulling the victim to the concrete floor by grabbing him by the belt. Resuscitation efforts were made by the foreman. A detached 220 volt conductor was later found in the panel box. It was thought that the victim may have come into contact with the conductor while trying to maintain his balance. Recommendations arising from this accident include: providing appropriate training commensurate with tasks that employees are expected to perform, and developing and implementing a comprehensive occupational safety program for this facility.
