52-Year-Old First Class Electrician Electrocuted in Indiana

Description:A 52 year old first class electrician was electrocuted while performing preventive maintenance on a 34.5 kilovolt oil circuit breaker (OCB). The victim worked for a public utility company employing over 2200 people. The company has a written safety policy and program. The insulation values of the OCB were to be tested as part of the maintenance program. The victim moved the test van so he and his coworker could test the bushings. Meanwhile, the assistant engineer proceeded to the bus side of the OCB and opened the handle of the gang operated disconnects for the OCB. He visually checked to ensure that the disconnect blades had been opened. He placed a hold card on the handle. The workers tested the first two bushings on the OCB. The assistant indicated he was ready to test the third. The victim removed the lead from the second and as he touched the lead to the third bushing he was electrocuted. Investigation indicated that one of the disconnect blades remained closed after the handle for the gang operated disconnects had been opened. A broken porcelain insulator skirt at the base of the cap allowed the cap to remain stationary while the rest of the mechanism rotated when the handle was pulled. Recommendations arising from this accident include devising additional steps to verify the status of the equipment, training employees in both safe job procedures and hazard recognition, and including employee representatives in monthly safety meetings.
