Data-Driven Systems Change and Improvement in Hypertension Management

  Blood pressure control is a significant, modifiable risk factor in preventing heart disease and stroke, and yet many patients’ hypertension remains uncontrolled, and many others with high blood pressure have not yet been diagnosed and treated. Comprehensively addressing hypertension management should extend beyond care for individual patients and into changing organizational policies and processes to improve overall outcomes. NACDD’s Cardiovascular Health Initiative shares resources and program information to help states improve identification and control of hypertension. In coordination with CDC’s Division for Heart Disease and Stroke Prevention, this NACDD Initiative hosts fireside chats and other learning opportunities with a focus on hypertension, including this fourth fireside chat and the follow up virtual roundtables that focused on health systems change through a partnership with the Massachusetts Department of Public Health and its Health Center Controlled Network, the Massachusetts League of Community Health Centers. ; This publication (journal article, etc.) was supported by the Grant or Cooperative Agreement Number 5NU38OT000225-05, funded by the Centers for Disease Control and Prevention.
