  Investments in Community Health ; Community Transformation Grant Program (CTG) ; In 2011, the Community Transformation Grant Program (CTG) awarded approximately $103 million to 61 states and local government agencies, tribes and territories, and nonprofit organizations in 36 states, along with nearly $4 million to six national networks of community-based organizations. These Community Transformation Grants are expected to run for five years and benefit more than one in three U.S. citizens —approximately 120 million Americans.
