Description:Chronic diseases such as heart disease and stroke, cancer, and diabetes are leading causes of morbidity and mortality in the United States. Chronic diseases are also leading drivers of the nation’s annual $3.8 trillion in health care costs. Racial, ethnic, and socioeconomic chronic disease disparities persist, and they are bolstered by diferences in social determinants of health (SDOH) , “the conditions in the environments where people are born, live, learn, work, play, worship, and age that afect a wide range of health, functioning, and quality-of-life outcomes and risks.”
