    In 2014, little was known about the environmental and policy supports for healthy diets and regular physical activity in communities across the United States, and how these supports were changing. In 2014, the Centers for Disease Control and Prevention (CDC) conducted the first nationally representative survey of communities, Community Based Survey of Supports for Healthy Eating and Active Living (CBS HEAL) to address this gap. This survey documented the number and kind of policies and practices communities implement to support healthy eating and active living and community characteristics. Policies and practices assessed included CDC’s program strategies and recommendations as well as those from expert groups, such as the Institute of Medicine (IOM).

    In 2021, CDC conducted CBS HEAL again to update the information and document changes in policies and practices since 2014. Surveillance of these supports will help guide efforts to improve diet and physical activity behaviors and can help state and local governments develop community interventions, monitor progress of communities in improving these supports, and evaluate related interventions.

    CDC contracted with NORC at the University of Chicago on this 2021 survey (OMB control number 0920-1327, expiration date 03/31/2023). Data collection for CBS HEAL began in May 2021 and concluded in September 2021. This report provides information about the methods used for this national survey of municipal governments. This data collection was deemed non- human subjects research by CDC and NORC.
