Fatality Assessment and Control Evaluation (FACE) Program

U.S. workplace deaths remain a public health crisis, with over 5,000 annual fatalities concentrated in high-risk sectors like construction, transportation, and agriculture. To address this under the OSH Act, NIOSH runs the Fatality Assessment and Control Evaluation (FACE) program, a non-regulatory initiative providing safety guidance through independent, non-enforcement investigations. Structurally, FACE operates as a unified effort split into two distinct, complementary branches: a centralized, national In-House program run directly by NIOSH, and a decentralized network of State FACE programs funded via cooperative agreements. While both share common goals and methods, they differ significantly in their organizational structure, authority, influence, and how they implement findings locally across diverse regional industries. Together, they generate extensive prevention resources and adapt to emerging workplace hazards.

The decedent was a member of a 4-person inmate work crew contracted by the County Drain Commission through local law enforcement. The work crew was clearing a county drain of trees. The decedent and his coworker (Inmate 1) had just felled a 24-inch diameter cotton wood tree. Inmate 1 operated the chain saw and the decedent inserted the wedge to keep the tree from sitting back on the chain. The cottonwood tree fell in its desired direction (northeast), and also knocked several trees over as it fell to the ground. The inmates and the Drain Commission crew leader indicated the ground shook when the cottonwood landed. Approximately 10-20 seconds later, the elm snag broke approximately 12 inches above the ground fell to the northwest. The snag broke into two pieces, with the 4-inch diameter uppermost piece striking the decedent on the right side of his head. The decedent was not wearing a hard hat. An emergency response call was initiated by one of the inmates, and another inmate began resuscitative efforts. Emergency response arrived and assumed care of the decedent. He was transported to a local hospital where he was declared dead. Factors contributing to this incident include: 1) Inadequate site assessment for hazard trees AB - 2) Lack of personal protective equipment use AB - and 3) Inadequate training regarding tree felling. RECOMMENDATIONS/DISCUSSION: 1) Crew supervisors and tree fellers should receive training to properly evaluate the trees and surrounding area so potential hazards can be identified and appropriate control measures implemented. Training should be a continuing process for skills development and for the understanding of safe methods and practices in the logging industry. 2) A "pre-job safety plan" should be in place for the cutting site and the plan should be reviewed prior to each day's cutting. 3) Correctional facilities with an inmate work crew program should develop, implement, and enforce a written health and safety program for the inmates, which includes, but is not limited to, training in hazard identification and mitigation, personal protective equipment and methods for dealing with inmate noncompliance of the health and safety program requirements, such as unannounced inspections. 4) Drain Commissions should develop, implement, and enforce a written health and safety program for its employees which includes, but is not limited to, training in hazard identification, avoidance, and abatement for the work performed by them as well as methods for dealing with employee noncompliance of the health and safety program requirements. Although not causative factors in this incident, MIFACE makes the following recommendations to minimize the possibility of a future work-related fatality: 1) All tree cutting operations should adhere to the principle that a distance of at least two tree lengths should separate adjacent occupied work areas. 2) The area surrounding an incident site that has standing trees which pose a hazard to rescue and/or investigative personnel should be felled if possible. If unable to be felled, an individual should be assigned as a spotter to alert rescue/investigative personnel to any change in the tree(s) stability. 3) Investigative agencies, such as Police, Sheriff and State Police should have hard hats and other personal protective equipment readily available for protection against hazards present at the incident scene. The employee was assisting the foreman by moving insulation to the slightly tapered roof drainage areas. The roof hatch's curb and cover had been installed by another contractor. The sequence of events leading to the fall was unwitnessed. At some point, the decedent fell onto the sheet metal cover. The cover gave way and both the cover and the decedent fell 48 feet to the concrete floor below. Emergency response was called and the decedent was declared dead at the scene. No fasteners were found on the sheet cover, ground or on the roof after the incident occurred. The hatch cover had no tears or other abrasions as a result of the decedent's fall. Three other employers (general contractor, primary mechanical contractor, and subcontractor of the primary mechanical contractor) were working onsite when the fatality occurred. MIOSHA cited all four employers of the multi-employer worksite at the conclusion of its investigation. Key contributing factors identified in this investigation were: 1) Improper materials used and securement of the cover for access hatch AB - 2) Company's Environmental Health and Safety program requirements were not followed AB - and 3) Construction companies interaction with each other. RECOMMENDATIONS: 1) Construction employers should develop and ensure implementation of their MIOSHA-required accident prevention program at the jobsite including, but not limited to, daily hazard assessments to identify and mitigate hazards, such as unsecured roof covers, to ensure employee safety. The truck was not the unit that the victim usually operated. 2. Slippery/muddy conditions. 3. Worker was outside cab with Power Take Off (PTO) on. NJ FACE investigators recommend that these safety guidelines be followed to prevent similar incidents: 1. Roll-off truck operators should only use the external hydraulic controls when absolutely necessary and if so, with extra caution. If an operator is not intending on utilizing the external controls and is exiting the cab, the Power Take-Off (PTO) should be disengaged. 2. All parts and equipment on the roll-off truck should meet appropriate specifications as specified in 49 CFR 393.203. 3. A safety and health plan based on a job hazard analysis should be developed by the employer and followed where workers are assigned tasks. Department of Public Health AB - On October 18, 2011 a 58-year-old male municipal maintenance technician/electrician (victim) was fatally injured when he fell from a vehicle-mounted aerial lift's raised bucket. The victim and two co-workers were repairing a faulty cantilevered traffic signal at a four-way intersection. The victim was inside the raised bucket accessing the traffic light when a tractor-trailer driving through the intersection struck the raised bucket. The victim was ejected out of the bucket and fell approximately seventeen feet to the roadway below. A call was placed for Emergency Medical Services (EMS) by the co-workers. Within minutes EMS and the local police arrived at the incident location and the victim was transported to a local hospital where he was pronounced dead. The Massachusetts FACE Program concluded that to prevent similar occurrences in the future, municipalities should: 1. Ensure that fall protection is used when working from an aerial lift truck's raised bucket/platform AB - 2. Ensure that when performing work in roadways that work zones are set up, at a minimum, in accordance with the Manual on Uniform Traffic Control Devices (MUTCD), Part 6, developed by the U.S. Department of Transportation Federal Highway Administration AB - 3. Ensure that employees' exposure to moving traffic is minimized when working in and around roadways by developing temporary traffic control plans, including never allowing traffic to pass underneath raised aerial lift buckets/platforms AB - 4. Provide and ensure that employees wear the appropriate personal protective equipment, including American National Standard Institute (ANSI) compliant high visibility safety apparel, when working along roadways AB - 5. Provide work zone safety training for all employees who will be required to complete tasks while in proximity to roadways AB - 6. Ensure that each department develops, implements, and enforces a comprehensive health and safety program that includes hazard recognition and avoidance of unsafe conditions AB - 7. The victim was operating a skid steer loader to remove demolition debris on the building's 21st floor. On the floor above him remote-controlled demolition machines were making holes in the floor. As these machines worked, cables, chunks of concrete, and rebar were left hanging. Three laborers were using propane torches to cut the hanging cables and rebar while the victim and another worker used skid steer loaders to scoop up the debris and dump it down a chute. As the victim was operating the skid steer loader he was impaled in the neck by a 7 foot long ½ inch wide piece of rebar suspended from the ceiling. He was able to back up the machine and shut it down. A fellow laborer applied pressure to his neck. Emergency medical services were called and rendered first aid. The victim died at the scene. The tank was a permit-required confined space. The space was not adequately ventilated and the victim was not trained in confined space entry. There was no attendant at the tank opening to monitor the work process while the victim was in the tank. The victim was wearing a cartridge respirator that did not adequately protect against inhaling methylene chloride vapors. The victim was observed unresponsive at the bottom of the tank by a co-worker. The co-worker tried to rescue the victim and was overcome by vapors. The high concentration of methylene chloride in the product, the tank configuration, the inadequate ventilation, and the inadequate training and implementation of confined space procedures were contributing factors in this incident. The CA/FACE investigator determined that in order to prevent exposure to methylene chloride while cleaning paint tanks, employers should ensure that: 1. Policies and procedures are developed and implemented to clean paint tanks more frequently with water-based materials before the paint is cured. If this is not possible, the cured paint should be stripped with abrasive removal methods. 2. If toxic chemicals must be used inside a tank, employees must be provided with worker training in chemical hazard communication and confined space entry procedures. When his wife did not come out of the milking area after starting a group of cows on the milking machines, the husband went into the parlor to check on her. He found her lying near the gate between the holding pen and the milking area. She was unable to move, and told her husband the bull in the holding pen had struck her from behind. The husband moved her into the milking area, secured the cattle in the holding pen, and called 911. First responders arrived and transported the woman to the hospital. The woman was awake and oriented upon arrival but in critical condition with multiple blunt-force injuries and a near-severed spinal cord. Her condition deteriorated, and she died three days later. The factors contributing to this fatal injury include working alone, and working in an enclosed area where a bull was in the presence of one or more cows likely in estrus. The following recommendations are made to prevent future occurrences: 1. Maintain constant awareness of the location and behavior of bulls when entering a holding area or pasture. Work with a partner when possible, rather than alone. 2. Recognize scenarios and behaviors indicating that bulls are likely to threaten or attack. 3. Cull bulls from the herd upon first display of aggressive or frenzied behavior toward handlers. In addition to the specific recommendations above related to this case, dairy producers should warn visitors and others unfamiliar with dairy bulls' potentially aggressive behavior to not enter pastures, holding pens, or corrals where livestock are present. When planning dairy facilities, include a secure holding facility where bulls can be routed and held during milking, and accessible escape routes or protected areas such as passage gates for working in close quarters with livestock. If economically feasible, consider using artificial insemination to eliminate the need for dairy bulls on site. One end of the rope caught in the drum of the wood chipper and became taut, tangled around the victim, and pulled him onto the wood chipper feed table. The victim was then decapitated by the rope. Factors that may contribute to worker death when operating a wood chipper include feeding brush or branches which contain foreign objects into the chipper, and work procedures that required the use of a rope that present an entanglement hazard at the point of operation of the chipper. The CA/FACE investigator determined that, in order to prevent future incidents, land clearing companies that use wood chippers should: 1. Ensure that cut tree branches are placed in piles and checked for unwanted objects before being fed into wood chippers. 2. Assign a co-worker as a safety watch whenever cut tree branches are being fed into a wood chipper. Occupational Health Surveillance Unit AB - A 51-year-old male Hispanic construction worker died after a 40-foot fall from the roof at an industrial warehouse construction site. On the day of the incident, the decedent was installing corrugated metal roof decking. He was wearing fall protection equipment, which included a harness and a retractable lanyard connected to a metal anchor device. The anchor was attached to the unsecured leading edge of the corrugated decking. When the victim fell off the roof, the force caused the anchor to break free and the victim fell approximately 40 feet to the ground. Contributing Factors: 1. Improper anchor plate was used for the type of decking installed. 2. Improper screws used to fasten anchor plate. 3. Anchor plate was mounted on an unsecured edge. NJ FACE investigators recommend that these safety guidelines be followed to prevent similar incidents: 1. Utilize the appropriate type of anchor plate when installing corrugated roof decking. 2. Anchor plate screws should conform to manufacturer's specifications, including type and length of threading. 3. While installing roof decking, the anchor of the fall protection system should not be affixed to an unsecured leading edge. 4. A safety and health plan based on a job hazard analysis should be developed by the employer and followed where workers are assigned tasks. The victim exited the truck cab to close an overhead door of the regional recycling center (also operated by the victim's employer) after he and the driver had emptied a load of cardboard. The driver pulled out of the building with the truck's tailgate partly lowered to clear the doorway. He parked the truck and exited the cab to manually secure the tailgate in closed position using levers located on the outside of the driver's side of the truck. The victim, who was out of sight of the driver, spotted cardboard that had not completely cleared the tailgate area and attempted to pull it free at the same time the driver was lowering the tailgate. When the driver realized the tailgate was not closing properly, he raised it and discovered the victim had been caught between the lowered tailgate and the body of truck. Employees assisted the conscious victim to the office and drove him to a local hospital. He was transferred by helicopter to a regional medical center and died the following evening. After the fatality and prior to the Iowa FACE investigation, the employer proactively identified and implemented Recommendations 1 through 6 below to protect employees. RECOMMENDATIONS: 1. Demarcate restricted areas to keep all persons a safe distance from moving trucks and unloading activities. 2. Designate trained spotters to direct drivers in all truck movement when entering, unloading, and exiting facilities where employees or bystanders may be present. 3. Train all drivers - of both company trucks and non-company commercial vehicles involved in loading and unloading of materials - on the employer's safety policy regarding the use of spotters and truck traffic. Train all drivers to stop their vehicle when visual contact with the spotter is lost. 4. Require all employees and visitors to wear high-visibility vests or shirts in truck traffic and unloading areas. 5. Specify that only drivers are authorized to unsecure tailgate latches and operate tailgate mechanisms. 6. Require drivers to follow defined protocols for removing lodged materials from truck bodies or hoppers. These include using tailgate props to secure the tailgate in a supported open position, and shutting off the ignition and pocketing keys prior to clearing the tailgate or rear body of the truck. 7. Develop and implement specialized accommodations and training programs in businesses to support safety and health professionals, training specialists, employers of and employees with disabilities. At the end of the work day, the victim carried a tarp up an extension ladder to the roof so the owner could cover the roof in anticipation of impending rain. The victim fell an estimated 10 to 12 feet to the foundation retaining wall located to the left of the ladder. The fall was not witnessed. A stabilizer bar attached to the ladder had come apart from the ladder, and a nylon tie-down strap used to secure the stabilizer bar to the roof had broken when the ladder slid sideways. The owner of the business heard the victim fall and called 911. Local EMS responders arrived to find the victim unconscious with multiple blunt force injuries to the head, neck, torso, thorax, and abdomen. The victim was transported to a hospital approximately 30 miles away and died 4 hours later. RECOMMENDATIONS: 1. Use a hoist or pulley to raise and lower the heavy or awkward objects to their destination. Do not carry any objects or loads up a ladder that would cause an employee to lose balance. 2. Set up non-self-supporting extension ladders on firm level footing so height-to-base ratio is 4:1. Assure both rails of the ladder maintain equal contact with the supporting structure. Tie or stake the ladder so the top and bottom are secure and unable to move laterally. 3. Train employees who use ladders to recognize all fall hazards at the worksite and the means to eliminate those hazards. 4. Regularly check ladders and ladder accessories, including straps, stabilizers, clips or bolts affixing stand-off bars or stabilizers - to assure they are intact and not missing, damaged, or worn. 5. Use ladders that are sized for the maximum load and capacity that will be needed. Do not load ladders beyond the manufacturer's rated capacity. 6. Wear slip-resistant footwear when on ladders. Keep the midline of the body between the side rails of the ladder. Keep "three points of contact" on the ladder at all times. 7. Identify effective ways to effectively provide occupational safety resources and training to small special population business owners that may not have access to traditional safety resources such as trade organizations, insurance carriers, or OSHA consultation services. The bulldozer operator was able to exit the machine and drove his truck to a nearby field to get help from the property owner's son. The property owner's son called 911, but the dispatcher could not locate the address, so the property owner drove the injured man to a local hospital less than 20 miles away. The victim was then airlifted to a level 1 burn treatment center 60 miles away. Due to the extent and severity of his burns, he was taken off life support the following day, and died one day later. RECOMMENDATIONS: 1. Open fires should always be attended and supervised. Machinery should not be operated near open flames. 2. Keep a fully charged Class ABC-rated fire extinguisher in the cab or accessible storage area of farming and construction machinery. 3. Individuals should be aware of state safety policies for open burning and permit requirements for burns. 4. When burning trash or outdoor debris, contact the local fire department and advise them of your plans and location. 5. Plan for accessing emergency services. Individuals should contact local dispatchers to make sure adequate information is available to locate the residence or burn area. 6. Individuals should not work alone in risky conditions, such as when working with an open fire. When working alone in isolated areas, individuals should have plans to summon emergency assistance. 7. Machinery should be maintained in good condition and inspected before use, with specific attention to fuel lines and possible leaks. As the latch released, the corn head separated from the feeder and pinned him to the ground. It took first responders ten to fifteen minutes to lift the corn head off the victim, who was then transported to a Level I trauma care facility via helicopter. Efforts to resuscitate the victim were unsuccessful. RECOMMENDATIONS 1. Do not work under elevated equipment without blocking equipment to prevent its falling. 2. Confirm that all required steps are completed, in order, when working with heavy machinery. 3. Prior to using equipment, inspect machinery and ensure that safety warnings are visible. 4. Contact distributors to identify new safety features that may be available on existing equipment. He was seen by neighbors at approximately 11:45 AM, but when he did not arrive for lunch at 12:30 PM, his wife went out to the field and found him at the baler with both arms caught in the compression rollers. She ran to the house of a neighbor who called 911. Another neighbor, who was an emergency responder, heard the call on his scanner AB - he and his son arrived at the site and turned off the tractor and power takeoff (PTO), but the victim remained trapped. Rescue personnel arrived and while attempting to remove the victim determined he was not breathing. Rescue operations were stopped at 1:10 PM. RECOMMENDATIONS 1. Always disengage the PTO, set the brake, shut off the tractor, and pocket the key when dismounting a tractor that is attached to a baler. 2. Machine equipment users should identify in-running nip point hazards and understand the risks and how to control access to these locations. 3. Identify original and retrofitted machine guards and safety devices for all farming equipment currently in use and make sure they are installed. 4. Identify original and updated safety bulletins for existing farm equipment. 5. Before operating a piece of machinery, review the hazards associated with the operation of the machine. On the day of the incident, the victim and a coworker were preparing to remove a canvas tarp covering stacked bales of alfalfa hay so that bales could be loaded onto a truck. The bales were approximately 1,500 lb, 3x4x8 foot, and stacked six high in two rows of twenty bales, with a center row placed on top. They were working on opposite sides of the stacks untying the anchor ropes used to secure the tarp. As the victim walked around the end of the stack, three bales fell from the end row, with one of them landing on top of him. A coworker used a materials handler to lift the bale from the victim. He was taken to a hospital where he was pronounced dead.
